摘 要:目的探讨高速涡轮手机联合微创拔牙治疗下颌阻生牙的效果。方法选取阳谷县人民医院口腔科2022年1月—2023年12月收治的120例下颌阻生牙患者作为研究对象,按照手术方式的不同分为微创组和对照组各60例。微创组采用高速涡轮手机联合微创拔牙,对照组采用凿骨劈冠拔除术。比较两组治疗情况、拔牙前后不同时间点疼痛情况、拔牙前后生活质量及术后并发症发生情况。结果两组治疗情况比较,微创组与对照组相比,手术时间更短、术中出血量更少、牙窝完整性评分更高,差异有统计学意义(P<0.05)。微创组拔牙后1 d和3 d疼痛视觉模拟评分法评分均低于对照组,差异有统计学意义(P<0.05)。拔牙后两组生活质量评分均高于拔牙前,且拔牙后微创组生活质量评分高于对照组,差异有统计学意义(P<0.05)。微创组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论采用高速涡轮手机联合微创拔牙治疗下颌阻生牙,能够减轻患者疼痛,提高生活质量,降低并发症发生率,具有较为理想的治疗效果。Objective To explore the effect of high-speed turbine handpieces combined with minimally invasive tooth extraction in the treatment of impacted mandibular teeth.Methods A total of 120 patients with impacted mandibular teeth admitted to the Department of Stomatology at Yanggu County People’s Hospital from January 2022 to December 2023 were selected as the study subjects.They were divided into a minimally invasive group and a control group according to different surgical method,with 60 patients in each group.The minimally invasive group received high-speed turbine handpieces combined with minimally invasive tooth extraction,while the control group received chisel osteotomy.Compare the treatment outcomes and pain levels at different time points and quality of life before and after tooth extraction,as well as the occurrence of postoperative complications after tooth extraction.Results The comparison of treatment outcomes between the two groups showed that the minimally invasive group had a significantly shorter surgical time,less intraoperative bleeding,and a higher score for tooth socket integrity compared to the control group(P<0.05).The visual analogue scores of pain in the minimally invasive group were significantly lower than those in the control group on the 1st day and 3rd day after tooth extraction(P<0.05).After tooth extraction,the quality of life scores of both groups were significantly higher than before,and the quality of life scores of the minimally invasive group were significantly higher than those of the control group(P<0.05).The total incidence of complications in the minimally invasive group was significantly lower than that in the control group(P<0.05).Conclusion The use of high-speed turbine handpieces combined with minimally invasive tooth extraction can effectively alleviate patient pain,improve quality of life,reduce the incidence of complications,and have a relatively ideal treatment effect in the treatment of impacted mandibular teeth.
