血栓弹力图、脂蛋白相关磷脂酶A2与急性大动脉粥样硬化性脑梗死预后相关性分析

摘　　要：目的分析血栓弹力图(TEG)参数、血清脂蛋白相关磷脂酶A2(Lp-PLA2)水平与急性大动脉粥样硬化(LAA)性脑梗死预后的相关性。方法选取2023年1月—2024年5月本院收治的87例LAA性脑梗死患者为研究对象,根据美国国立卫生研究院卒中量表(NIHSS)评分将LAA性脑梗死患者分为轻度组(62例)、中度组(25例);3个月后,使用改良Rankin量表(mRS)评分评估患者预后。比较不同程度脑损伤以及不同预后患者TEG参数(K值、R值、Ma值、Angle)、血清Lp-PLA2水平。采用pearson相关性分析TEG参数、Lp-PLA2水平与mRS评分的相关性。结果Ma值、Angle以及Lp-PLA2水平由低到高依次为轻、中组,K值、R值由低到高依次为中、轻度组,两组相比差异有统计学意义(P<0.05);与预后良好组相比,预后不良组Ma值、Angle以及Lp-PLA2水平较高,K值、R值较低(P<0.05)。Pearson相关性分析显示,Ma值、Angle、Lp-PLA2水平与mRS评分呈正相关(r=0.558,0.574,0.619,P均<0.05),K值、R值与mRS评分呈负相关(r=-0.492,-0.482,P均<0.05)。结论LAA性脑梗死患者TEG参数、血清Lp-PLA2水平与LAA性脑梗死预后密切相关,可以使用上述指标评估患者预后。Objective To analyze correlation between thromboelastography(TEG)parameters,serum lipoprotein associated phospholipase A2(Lp-PLA2)levels and prognosis of acute cerebral infarction of large artery atherosclerosis(LAA).Methods The paper chose 87 patients with cerebral infarction of LAA in our hospital from January 2023 to May 2024 as research subjects,and divided them into mild group(62 cases)and moderate group(25 cases)according to the National Institutes of Health Stroke Scale(NIHSS)scores.After 3 months,prognosis of patients was evaluated with the Modified Rankin Scale(mRS)scores.TEG parameters(K value,R value,MA value,Angle)and serum Lp-PLA2 level of patients with different degrees of brain injury and prognoses were compared.Correlation between TEG parameters,Lp-PLA2 level and mRS scores was analyzed with pearson correlation analysis.Results MA value,Angle,and Lp-PLA2 level of mild and moderate group was from low to high,while K and R value of moderate and mild group was from low to high.Difference between two groups was statistically significant(P<0.05).The group with poor prognosis had higher MA value,Angle and Lp-PLA2 level,and lower K and R values than that with good prognosis,(P<0.05).Pearson correlation analysis showed,MA value,Angle and Lp-PLA2 level was positively correlated with mRS scores(r=0.558,0.574,0.619,all P<0.05),while K value and R value was negatively correlated with mRS scores(r=-0.492,-0.482,all P<0.05).Conclusion TEG parameters and serum Lp-PLA2 level are closely related to prognosis of cerebral infarction of LAA,and above indicators can be used to evaluate prognosis of patients.
