术前淋巴结超声影像特征对甲状腺癌颈侧区淋巴结转移风险的预测

摘　　要：目的分析术前颈部淋巴结的超声影像特征,探索其与甲状腺癌颈侧区淋巴结转移之间的联系。方法选取2018年1月1日至2024年8月1日于安徽医科大学第二附属医院就诊的296例甲状腺癌患者。颈侧区淋巴结术后病理阳性患者纳入观察组(260例),病理阴性患者纳入对照组(36例)。分析术前可疑淋巴结的超声影像特征,探索甲状腺癌颈侧区淋巴结转移的影响因素。结果单因素分析结果显示可疑淋巴结的大小、区域多发、皮髓质分界异常、存在微钙化、形状不规则、存在异常血流信号与颈侧区淋巴结转移之间存在相关性(P<0.05),多因素Logistic回归分析显示超声提示可疑淋巴结微钙化、存在高回声团、存在异常血流信号是颈侧区淋巴结转移的独立风险因素(P<0.05)。结论甲状腺癌颈侧区淋巴结的术前超声影像特征对淋巴结是否发生转移有较好的预测效果。Objective To explore the relationship between preoperative ultrasonographic features of cervical lymph nodes and lateral neck lymph node metastasis in thyroid cancer by analyzing these characteristics.Methods A total of 296 patients with thyroid cancer who visited the Second Affiliated Hospital of Anhui Medical University from January 1,2018 to August 1,2024 were selected.Patients with pathological positive results after cervical lymph node surgery were included in observation group(260 cases),while patients with pathological negative results were included in control group(36 cases).Analyze the ultrasound imaging characteristics of suspected lymph nodes before surgery and explore the influencing factors of lymph node metastasis in the cervical region of thyroid cancer.Results The results of univariate analysis showed that there was a correlation(P<0.05)between the size and frequency of suspicious lymph nodes,abnormal boundaries between the cortex and medulla,presence of microcalcifications,irregular shapes,and abnormal blood flow signals and lateral cervical lymph node metastasis.Multivariate Logistic regression analysis showed that ultrasound findings of suspicious lymph node microcalcifications,presence of hyperechoic masses,and presence of abnormal blood flow signals were independent risk factors for cervical lymph node metastasis(P<0.05).Conclusion The preoperative ultrasound imaging features of cervical lymph nodes in thyroid cancer have a good predictive effect on whether lymph nodes have metastasized.
