某高校校医院门诊不合理抗生素处方分析

摘　　要：目的点评某高校校医院门诊抗生素处方,分析抗生素使用存在的问题及原因,提出改进措施。方法回顾性调查长江师范学院医院2023年6月—2024年6月门诊抗生素处方,随机选取抗生素相关处方1400张,结合门诊病历记录,对选取处方进行点评、分析,对不合理处方进行分类、统计。结果1400张门诊抗生素处方中,存在不合理处方121张。其中,不规范处方25张(20.66%,25/121),以超量处方(9张)和诊断、用药不符(8张)为主,合计占不规范处方的68.00%(17/25);用药不适宜处方78张(64.46%,78/121),以用法用量不合理(40张)、抗生素适应证不适宜(17张)和抗生素联用不合理(12张)为主,合计占用药不适宜处方的88.46%(69/78);超常处方18张(14.88%,18/121),以无适应证用药为主,占超常处方的66.67%(12/18)。结论门诊不合理使用抗生素问题较多,用药不适宜处方占比较大,在药物用法用量、抗生素适应证、抗生素联用等方面问题较多,医院需加强医师、药师临床药物相关知识培训学习和监督考核,促进临床合理使用抗生素。Objective To evaluate the antibiotic prescriptions in the outpatient department of a university hospital,analyze the problems and causes in the use of antibiotics,and propose improvement measures.Methods A retrospective investigation was conducted on the outpatient antibiotic prescriptions in the Hospital of Yangtze Normal University from June 2023 to June 2024,a total of 1400 antibiotic-related prescriptions were randomly selected,combined with the outpatient medical records,the selected prescriptions were evaluated and analyzed,and the irrational prescriptions were classified and statistically analyzed.Results There were 121 unreasonable prescriptions in 1400 outpatient antibiotic prescriptions,among them,25 prescriptions(20.66%,25/121)were non-standard prescriptions,mainly excessive prescriptions(9 prescriptions)and inconsistent diagnosis and medication(8 prescriptions),accounting for 68.00%(17/25)of non-standard prescriptions;there were 78 inappropriate prescriptions(64.46%,78/121),mainly including unreasonable usage and dosage(40 prescriptions),inappropriate indications of antibiotics(17 prescriptions)and unreasonable combination of antibiotics(12 prescriptions),accounting for 88.46%(69/78)of inappropriate prescriptions;there were 18 supernormal prescriptions(14.88%,18/121),mainly non-indication medication,accounting for 66.67%(12/18)of supernormal prescriptions.Conclusions There are many problems in the irrational use of antibiotics in the outpatient department,with a relatively high proportion of inappropriate prescriptions,there are many problems in drug dosage and administration,antibiotic indications,and antibiotic combination,hospitals need to strengthen the training and learning of clinical drug-related knowledge and supervision and assessment for physicians and pharmacists to promote the rational use of antibiotics in clinical practice.
