摘  要:目的观察^(18)F-前列腺特异性膜抗原(PSMA)-1007 PET/MRI诊断前列腺癌(PCa)侵犯精囊腺(SVI)的价值。方法回顾性纳入92例接受根治性前列腺癌切除术的男性PCa患者,根据术后病理所见有无SVI将其分为阳性组(n=26)与阴性组(n=66)。比较组间PCa病灶PET/MRI参数,包括最大标准摄取值(SUV_(max))、最小表观弥散系数(ADC_(min))、平均表观弥散系数(ADC mean)、SUV_(max)/ADC_(min)、SUV_(max)/ADC mean、PSMA肿瘤体积(PSMA-TV)及PSMA表达总量(TL-PSMA);绘制受试者工作特征曲线,评估各参数诊断SVI的效能。结果92例中,^(18)F-PSMA-1007 PET/MRI显示30例SVI、62例无SVI,其诊断SVI的准确率为73.91%、敏感度为61.54%、特异度为78.79%、阳性预测值为53.33%、阴性预测值为83.87%。组间ADC_(min)、PSMA-TV及TL-PSMA差异均有统计学意义(P均<0.05)。以SUV_(max)、ADC_(min)、ADC mean、SUV_(max)/ADC_(min)、SUV_(max)/ADC mean、PSMA-TV及TL-PSMA诊断PCa SVI的曲线下面积(AUC)分别为0.554、0.341、0.396、0.603、0.581、0.755及0.705;其中,PSMA-TV的AUC高于除TL-PSMA外其他参数,其敏感度为84.60%、特异度为56.10%。结论^(18)F-PSMA-1007 PET/MRI有助于诊断PCa SVI。Objective To observe the value of^(18)F-prostate specific membrane antigen(PSMA)-1007 PET/MRI for diagnosing seminal vesicle invasion(SVI)of prostatic cancer(PCa).Methods Totally 92 male patients with PCa who underwent radical prostatectomy were retrospectively enrolled and divided into positive group(n=26)and negative group(n=66)based on postoperative pathology showed SVI or not.PET/MRI parameters,including_(max)imum standard uptake value(SUV_(max)),minimum apparent diffusion coefficient(ADC_(min)),mean apparent diffusion coefficient(ADC mean),SUV_(max)/ADC_(min),SUV_(max)/ADC mean,PSMA tumor volume(PSMA-TV)and total lesion PSMA(TL-PSMA)were compared between groups.The receiver operating characteristic curve was drawn,and the efficacy of each parameter for diagnosing SVI was analyzed.Results Among 92 cases of PCa,^(18)F-PSMA-1007 PET/MRI showed 30 cases with SVI and 62 cases without SVI,with accuracy of 73.91%,sensitivity of 61.54%,specificity of 78.79%,positive predictive value of 53.33%and negative predictive value of 83.87%.Significant differences of ADC_(min),PSMA-TV and TL-PSMA were found between groups(all P<0.05).The area under the curve(AUC)of SUV_(max),ADC_(min),ADC mean,SUV_(max)/ADC_(min),SUV_(max)/ADC mean,PSMA-TV and TL-PSMA for diagnosing SVI of PCa was 0.554,0.341,0.396,0.603,0.581,0.755 and 0.705,respectively.The AUC of PSMA-TV was higher than other parameters except for TL-PSMA,with sensitivity of 84.60%and specificity of 56.10%.Conclusion^(18)F-PSMA-1007 PET/MRI was helpful for diagnosing SVI of PCa.
