摘 要:目的探究生长分化因子15(GDF15)、甘露聚糖结合凝集素丝氨酸肽酶-2(MASP-2)在膀胱癌中的表达,并分析两者与淋巴结转移的关系。方法选取2018年2月至2022年11月在本院诊治的110例膀胱癌患者作为研究对象,根据淋巴结转移情况将患者分为转移组(42例)和未转移组(68例)。采用ELISA法检测患者的血清GDF15、MASP-2水平;采用logistic回归分析膀胱癌淋巴结转移的影响因素;采用受试者工作特征(ROC)曲线分析血清GDF15、MASP-2预测膀胱癌淋巴结转移风险的价值。结果与未转移组比较,转移组患者的血清GDF15水平降低,MASP-2水平升高(均P<0.05)。logistic回归分析结果显示,MASP-2和肿瘤分化程度是发生淋巴结转移的危险因素,血清GDF15是发生淋巴结转移的保护因素(均P<0.05)。ROC曲线分析结果显示,GDF15、MASP-2联合预测膀胱癌淋巴结转移的曲线下面积(AUC)大于血清GDF15单独检测(Z=1.984,P=0.047)及MASP-2单独检测(Z=2.315,P=0.021)。结论膀胱癌淋巴结转移患者的血清GDF15水平降低,MASP-2水平升高,早期监测患者的血清GDF15、MASP-2水平对预测膀胱癌淋巴结转移有一定价值。Objective To investigate the expression of growth differentiation factor 15(GDF15)and mannose-binding lectin associated serine protease-2(MASP-2)in bladder cancer,and their relationship with lymph node metastasis of bladder cancer.Methods A total of 110 patients with bladder cancer diagnosed and treated in our hospital from February 2018 to November 2022 were taken as the research objects.These patients were grouped into metastatic group(42 cases)and non metastatic group(68 cases)based on their lymphatic metastasis.Serum levels of GDF15 and MASP-2 were detected by enzyme linked immunosorbent assay(ELISA).Factors affecting lymph node metastasis in bladder cancer were analyzed by univariate and multivariate logistic regression,and the value of serum GDF15 and MASP-2 in predicting the risk of lymph node metastasis in bladder cancer was analyzed by recevier operator characteristic(ROC)curve.Results Compared with the non-metastatic group,serum GDF15 levels decreased and MASP-2 levels increased in the metastatic group(all P<0.05).Logistic regression analysis showed that MASP-2 and the degree of tumor differentiation were risk factors for lymph node metastasis,and serum GDF15 was protective factor for lymph node metastasis(all P<0.05).ROC curve analysis showed that the area under the curve(AUC)of GDF15 and MASP-2combined in predicting lymph node metastasis of bladder cancer was significantly greater than that of serum GDF15 alone(Z=1.984,P=0.047)and MASP-2 alone(Z=2.315,P=0.021).Conclusions The level of serum GDF15 in patients with lymph node metastasis of bladder cancer reduces,the level of MASP-2 increases,early monitoring of serum GDF15 and MASP-2 levels in patients with bladder cancer is of certain value in predicting lymph node metastasis.
