针刺百会及四神聪联合达郁安神汤剂对抑郁症患者睡眠质量的影响研究

摘　　要：目的分析针刺百会、四神聪联合达郁安神汤剂对抑郁症患者睡眠质量的影响。方法回顾性选取新乡医学院第二附属医院2022年1月—2024年1月收治的104例抑郁症患者的病历资料,根据不同的治疗方法分为对照组、观察组,各52例。对照组采用达郁安神汤剂治疗,观察组采用针刺百会、四神聪联合达郁安神汤剂治疗。比较两组的临床疗效,匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)、抑郁和躯体症状量表(depression and somatic symptoms scale,DSSS)、汉密尔顿焦虑量表(Hamilton anxiety scale,HAMA)评分及神经递质[5-羟色胺(5-hydroxytryp tamine,5-HT)、谷氨酸(glutamic acid,Glu)、多巴胺(dopamine,DA)]水平。结果观察组的临床总有效情况高于对照组(χ^(2)=4.300,P=0.038);治疗后,观察组的PSQI评分(睡眠质量、睡眠效率、入睡时间、睡眠时间、睡眠障碍、日间功能障碍)均低于对照组(t=11.740、8.197、7.907、8.174、7.413、9.343,均P<0.05);治疗后,观察组的DSSS评分、HAMA评分均低于对照组(t=9.952、7.886,均P<0.05);治疗后,观察组的5-HT、Glu、DA水平均高于对照组(t=5.702、4.558、5.855,均P<0.05)。结论针刺百会、四神聪联合达郁安神汤剂治疗抑郁症疗效确切,可减轻患者的抑郁、焦虑症状,调节神经递质表达,改善患者的睡眠质量。Objective To analyze the effect of acupuncture at Baihui,Sishencong and Dayu Anshen Decoction on sleep quality in patients with depression.Methods The data of 104 patients with depression treated in the Second Affiliated Hospital of Xinxiang Medical University from January 2022 to January 2024 were retrospectively collected,and they were divided into control group and observation group according to different treatment methods,with 52 cases in each group.The control group was treated with Dayu Anshen Decoction,and the observation group was treated with acupuncture Baihui,Sishencong and Dayu Anshen Decoction.The clinical efficacy,Pittsburgh sleep quality index(PSQI)score,depression and somatic symptoms scale(DSSS)score,Hamilton anxiety scale(HAMA)score,and neurotransmitters[5-hydroxytryp tamine(5-HT),glutamic acid(Glu),dopamine(DA)]levels were compared between the two groups.Results The total clinical effectiveness in the observation group was higher than that in the control group(χ^(2)=4.300,P=0.038);post-treatment,PSQI scores(sleep quality,sleep efficiency,time to sleep,sleep duration,sleep disturbance,daytime dysfunction)were lower than those in the control group(t=11.740,8.197,7.907,8.174,7.413,9.343,all P<0.05);post-treatment,the DSSS score and HAMA score of the observation group were lower than that of the control group(t=9.952,7.886,all P<0.05);post-treatment,5-HT,Glu,DA levels in the observation group were higher than the control group(t=5.702,4.558,5.855,all P<0.05).Conclusion The efficacy of acupuncture Baihui and Si Shencong combined with Dayu Anshen Decoction in treating depression is precise,which can reduce the symptoms of depression and anxiety,regulate the expression of neurotransmitters,and improve the quality of sleep of patients.
