摘  要:目的分析microRNA-181a-5p(miR-181a-5p)在甲状腺乳头状癌(papillary thyroid carcinoma,PTC)患者中的表达及其临床意义。方法选取2021年8月—2022年2月郑州大学附属洛阳中心医院收治的经术后病理确诊的111例PTC患者为观察组,52例经术后病理确诊为甲状腺良性结节(benign thyroid nodule,BTN)患者为对照组,比较两组患者一般资料的差异。通过qRT-PCR法检测甲状腺结节组织miR-181a-5p的相对表达量,分析其与各临床指标的相关性,并在癌症基因组图集(the Cancer Genome Atlas,TCGA)数据库中验证。通过多因素logistic回归分析miR-181a-5p是否为PTC的独立危险因素。绘制受试者操作特征(receiver operating characteristic,ROC)曲线比较并分析miR-181a-5p与临床常用的超声积分法对PTC的预测效能。结果观察组与对照组的年龄、促甲状腺激素(thyroid stimulating hormone,TSH)水平差异均有统计学意义(t=4.464、2.421,均P<0.05),甲状腺影像报告和数据系统(thyroid imaging reporting and data system,TI-RADS)分级、纵横比、边缘规则度、是否为低回声方面差异有统计学意义(χ^(2)=21.280、8.205、10.771、7.893,均P<0.05)。在TCGA数据库以及本研究收集的两组患者的甲状腺结节组织中均提示观察组患者miR-181a-5p的相对表达量较对照组高(Z=-3.280、-9.672,均P<0.05)。miR-181a-5p高表达组为女性、TI-RADS分级>3级、超声积分>5分、点状强回声、纵横比、被膜侵犯、边缘不清晰、边缘不规则占比均较miR-181a-5p低表达组高(χ^(2)=4.017、11.835、12.086、17.793、4.771、7.024、3.860、10.955,均P<0.05),miR-181a-5p高表达组的年龄较对照组小(t=4.677,P<0.05)。多因素logistic回归分析结果显示:超声积分法(OR:7.330,95%CI:1.629~32.978)及miR-181a-5p(OR:1.378,95%CI:1.072~1.772)水平是PTC的独立影响因素(P<0.05)。ROC曲线分析结果显示,miR-181a-5p的曲线下面积(area under the curve,AUC)、灵敏度、特异度分别为0.893(95%CI:0.843~0.944Objective To analyze the expression and clinical significance of microRNA-181a-5p(miR-181a-5p)in patients with papillary thyroid carcinoma(PTC).Methods 111 patients with PTC diagnosed by postoperative pathology and admitted to Luoyang Central Hospital Affiliated to Zhengzhou University from August 2021 to February 2022 were selected as the observation group,and 52 patients with benign thyroid nodules(BTN)diagnosed by postoperative pathology were selected as the control group,and the differences in the baseline data of the two groups were compared.The relative expression of miR-181a-5p in thyroid nodule tissues was detected by qRT-PCR,and its correlation with various clinical indicators was analysed and validated in the Cancer Genome Atlas(TCGA)database.Whether miR-181a-5p was an independent risk factor for PTC was analysed by multifactorial logistic regression.Receiver operating characteristic(ROC)curve analysis was plotted to compare the predictive efficacy of miR-181a-5p with that of the ultrasound integral method commonly used in clinical practice for PTC.Results Compared with the control group,the observation group showed significant differences in age,thyrotropin(TSH)(t=4.464,2.421,all P<0.05),as well as in the thyroid imaging reporting and data system(TI-RADS)classification of nodules,aspect ratio,margin regularity,and whether they were hypoechoic as assessed by thyroid ultrasound(χ^(2)=21.280,8.205,10.771,7.893,all P<0.05).Both the TCGA database and the collected thyroid nodule tissues from the two groups indicated that the relative expression level of miR-181a-5p was higher in the observation group than in the control group(Z=-3.280、-9.672,all P<0.05).In the high-expression group of miR-181a-5p,the proportion of females,TI-RADS classification>3 grade,ultrasound score>5 points,punctate hyperechoic areas,aspect ratio,capsule invasion,unclear margins,and irregular margins were all higher compared to the low-expression group of miR-181a-5p(χ^(2)=4.017,11.835,12.086,17.793,4.771,7.024,3.860,10.955,all P<0.
