内镜下低温等离子射频消融术对早期声门型喉癌患者语音变化及声带黏膜恢复的影响

摘　　要：目的:探讨内镜下低温等离子射频消融术(PRFA)对早期声门型喉癌患者语音变化及声带黏膜恢复的影响。方法:回顾性分析2021年10月-2022年10月周口市中心医院收治的82例早期声门型喉癌患者的临床资料,依据治疗方法的不同将其分为对照组(41例)与观察组(41例)。对照组患者采用开放喉部分切除术治疗,观察组患者采用内镜下PRFA治疗。比较两组患者围手术期指标、语音变化、声带黏膜愈合评分、疼痛程度、嗓音功能、并发症及复发情况。结果:观察组患者手术及住院时间短于对照组,术中出血量少于对照组,术后观察组谐噪比高于对照组,基频微扰及振幅微扰低于对照组,术后声带黏膜愈合评分、视觉模拟评分法(VAS)评分及嗓音障碍指数量表(VHI)评分低于对照组,差异均有统计学意义(t=21.763、4.572、25.622、2.816、2.739、4.919、3.424、5.985、5.067,P<0.05);观察组患者并发症总发生率低于对照组,差异有统计学意义(χ^(2)=5.145,P<0.05);两组患者复发率比较,差异无统计学意义(χ^(2)=0.139,P>0.05)。结论:早期声门型喉癌患者采用内镜下PRFA治疗可促进声带黏膜恢复,改善患者语音功能,且损伤小、并发症少,术后复发率并未增加。Objective:To explore the effect of endoscopic low-temperature plasma radiofrequency ablation(RFA)on speech changes and vocal mucosal recovery in patients with early glottic carcinoma.Methods:A retrospective analysis was conducted on the clinical data of 82 patients with early glottic carcinoma treated in the hospital from October 2021 to October 2022.They were divided into control group(41 cases)and observation group(41 cases)based on different treatment methods.The control group received open partial laryngectomy treatment,while the observation group received endoscopic PRFA treatment.Perioperative indicators,speech changes,vocal mucosal healing score,pain level,voice function,complications,and recurrence between two groups were compared.Results:The duration of operation and hospitalization in the observation group was shorter than that in the control group,the amount of intraoperative blood loss was less than that in the control group,the harmonic to noise ratio of the observation group was higher than that in the control group,the fundamental frequency perturbation and amplitude perturbation were lower than that in the control group,and the scores of the postoperative vocal cord mucosal healing,visual analog scale(VAS)and voice disturbance index scale(VHI)were lower than that in the control group.The difference was statistically significant(t=21.763,4.572,25.622,2.816,2.739,4.919,3.424,5.985,5.067;P<0.05).The total incidence of complications in observation group was lower than that in control group,the difference was statistically significant(χ^(2)=5.145,P<0.05).There was no significant difference in recurrence rate between the two groups(χ^(2)=0.139,P>0.05).Conclusion:Early glottic carcinoma patients treated with endoscopic PRFA can promote vocal mucosal recovery,and improve speech function,with minimal damage,fewer complications,and no increase in postoperative recurrence rate.
