甲状腺乳头状癌组织中CD56、Ki-67的表达及其与临床病理特征的关系

摘  要:目的探讨甲状腺乳头状癌患者组织中神经细胞黏附分子56(CD56)、增殖抗原-67(Ki-67)的表达及其与临床病理特征的关系。方法选择2022年3月至2023年11月郑州大学第一附属医院收治的98例甲状腺乳头状癌患者为研究对象,取术中切除的癌组织及癌旁正常组织(距癌组织≥5 cm),采用免疫组织化学染色法检测所取组织中CD56、Ki-67的表达情况,分析不同病理特征甲状腺乳头状癌患者癌组织中CD56和Ki-67表达情况及其与临床病理特征的关系。结果甲状腺乳头状癌组织中Ki-67阳性表达率高于癌旁组织,CD56阳性表达率低于癌旁组织,差异有统计学意义(P<0.05)。不同临床分期、淋巴结转移、分化程度甲状腺乳头状癌组织中CD56、Ki-67表达比较,差异有统计学意义(P<0.05)。经卡方检验Phi系数分析,癌组织中CD56表达与临床分期、淋巴结转移呈负相关(Phi均<0,P<0.05),与分化程度呈正相关(Phi>0,P<0.05)。癌组织中Ki-67表达与分化程度呈负相关(Phi<0,P<0.05),与临床分期、淋巴结转移呈正相关(Phi均>0,P<0.05)。结论甲状腺乳头状癌患者组织中Ki-67阳性表达率高,CD56阳性表达率低,且与临床分期、分化程度、淋巴结转移相关。Objective To investigate the expression of neural cell adhesion molecule 56(CD56)and proliferative antigen-67(Ki-67)in papillary thyroid carcinoma and their relationship with clinicopathological features.Methods A total of 98 patients with papillary thyroid carcinoma admitted to the First Affiliated Hospital of Zhengzhou University from March 2022 to November 2023 were selected as the study objects.Intraoperative resection of cancer tissues and adjacent normal tissues(≥5 cm away from cancer tissues)were selected.Immunohistochemical staining was used to detect and compare the expressions of CD56 and Ki-67 in the tissues.The expression of CD56 and Ki-67 in the tissues of patients with papillary thyroid carcinoma and their relationship with different pathological features were analyzed.Results The positive expression rate of Ki-67 in papillary thyroid carcinoma tissues was higher than that in paracancer tissues,and the positive expression rate of CD56 in papillary thyroid carcinoma tissues was lower than that in paracancer tissues,with significant differnces(P<0.05).The expressions of CD56 and Ki-67 in papillary thyroid carcinoma tissues with different clinical stages,lymph node metastasis and differentiation were significant(P<0.05).Chi-square test Phi coefficient analysis showed that CD56 expression in cancer tissues was negatively correlated with clinical stage and lymph node metastasis(Phi<0,P<0.05),and positively correlated with differentiation(Phi>0,P<0.05).Ki-67 expression in cancer tissues was negatively correlated with the degree of differentiation(Phi<0,P<0.05),and positively correlated with clinical stage and lymph node metastasis(Phi>0,P<0.05).Conclusions The positive expression rate of Ki-67 is high and the positive expression rate of CD56 is low in the tissue of patients with papillary thyroid carcinoma,which is correlated with clinical stage,differentiation degree and lymph node metastasis.
