牙周炎患者血清HDAC3、sPD-1、Omentin-1水平与牙周指标及牙周-正畸联合治疗后预后的关系

摘　　要：目的探讨牙周炎患者血清组蛋白去乙酰化酶3(HDAC3)、可溶程序性死亡分子-1(sPD-1)、网膜素-1(Omentin-1)水平与牙周指标及牙周-正畸联合治疗后预后的关系。方法选取2018年9月至2020年5月在该院进行治疗的牙周炎患者102例作为研究组。另选取同期在该院进行牙周检查的健康者95例作为对照组。对牙周炎患者牙周-正畸联合治疗6个月后,依据复查结果预测预后情况分为预后不良组(n=39)、预后良好组(n=63)。采用酶联免疫吸附试验法检测血清HDAC3、sPD-1、Omentin-1水平。采用Pearson法分析牙周炎患者血清HDAC3、sPD-1、Omentin-1水平与牙周指标的相关性。受试者工作特征(ROC)曲线分析血清HDAC3、sPD-1、Omentin-1对牙周炎患者治疗后预后的预测价值。结果研究组血清sPD-1水平及牙周指标出血指数(SBI)、菌斑指数(PLI)、附着丧失(AL)、牙周探诊深度(PD)均显著高于对照组,而血清HDAC3、Omentin-1水平显著低于对照组,差异有统计学意义(P<0.05)。患者血清sPD-1水平与牙周指标SBI、PLI、AL、PD呈正相关,血清HDAC3、Omentin-1水平与牙周指标SBI、PLI、AL、PD呈负相关(P<0.05)。预后不良组治疗前PD、PLI、牙槽骨高度、血清sPD-1水平明显高于预后良好组,而血清HDAC3、Omentin-1水平明显低于预后良好组,差异有统计学意义(P<0.05)。ROC曲线分析结果显示,三者联合预测牙周炎患者治疗后预后的曲线下面积(AUC)优于血清HDAC3、sPD-1、Omentin-1各自单独预测的AUC(Z三者联合-HDAC3=2.754、Z三者联合-sPD-1=3.415、Z三者联合-Omentin-1=3.355,P=0.006、0.001、0.001)。结论牙周炎患者血清sPD-1水平明显升高,血清HDAC3、Omentin-1水平明显降低,且与牙周指标密切相关,三者联合对牙周炎患者治疗后预后具有较好的预测价值。Objective To investigate the relationship between serum histone deacetylase 3(HDAC3),soluble programmed death molecule-1(sPD-1)and Omentin-1 levels with periodontal indexes and prognosis after combined periodontal-orthodontic treatment in patients with periodontitis.Methods A total of 102 patients with periodontitis who were treated in a hospital from September 2018 to May 2020 were selected as the study group.Another 95 healthy subjects who underwent periodontal examination in the same period were selected as the control group.After 6 months of combined periodontal-orthodontic treatment,the patients were divided into poor prognosis group(n=39)and good prognosis group(n=63)according to the results of review.Serum levels of HDAC3,sPD-1 and Omentin-1 were detected by enzyme-linked immunosorbent assay.The correlation between serum HDAC3,sPD-1,Omentin-1 and periodontal indexes in patients with periodontitis was analyzed by Pearson method.The prognostic value of serum HDAC3,sPD-1 and Omentin-1 in patients with periodontitis after treatment was analyzed by receiver operating characteristic(ROC)curve.Results The serum levels of sPD-1,sulcus bleeding index(SBI),plaque index(PLI),loss of attachment(AL)and periodontal probing depth(PD)in the study group were significantly higher than those in the control group,while the serum levels of HDAC3 and Omentin-1 were significantly lower than those in the control group,with statistical significance(P<0.05).Serum sPD-1 levels were positively correlated with periodontal indicators SBI,PLI,AL and PD,while serum HDAC3 and Omentin-1 levels were negatively correlated with periodontal indicators SBI,PLI,AL and PD(P<0.05).Before treatment,PD,PLI,alveolar bone height and serum sPD-1 level in the poor prognosis group were significantly higher than those in the good prognosis group,while serum HDAC3 and Omentin-1 levels were significantly lower than those in the good prognosis group,with statistical significance(P<0.05).ROC curve analysis showed that the area under the curve(AUC)predicted th
