环孢素联合玻璃酸钠治疗中重度干眼症患者的临床效果

摘　　要：目的探究环孢素联合玻璃酸钠治疗中重度干眼症患者的临床效果。方法选取南京同仁医院2021年3月至2024年3月收治的中重度干眼症患者96例,根据治疗方法将患者分为对照组与观察组,各48例,对照组给予玻璃酸钠治疗,观察组给予环孢素联合玻璃酸钠治疗。对比两组治疗效果,治疗前后的眼表疾病指数(OSDI)评分、角膜荧光染色(CFS)评分、泪河高度(TMH)、泪膜破裂时间(BUT)、结膜充血程度及泪液分泌试验(SIT)变化情况。结果观察组治疗总有效率为93.75%(45/48)高于对照组79.17%(38/48)(χ^(2)=4.360,P<0.05)。治疗后,两组的CFS、OSDI、结膜充血程度评分均较治疗前降低,且观察组的CFS[(1.56±0.32)分vs(2.72±0.51)分]、OSDI[(20.91±4.36)分vs(27.28±4.42)分]、结膜充血程度评分[(1.12±0.25)分vs(1.39±0.32)分]均低于对照组(t=3.348、7.108、4.607,均P<0.05)。治疗后,两组的BUT、TMH、SIT均较治疗前升高,且观察组的BUT[(5.14±1.06)s vs(3.96±1.17)s]、TMH[(0.29±0.06)mm vs(0.20±0.05)mm]、SIT[(5.46±1.62)mm·5 min^(-1) vs(4.53±1.34)mm·5 min^(-1)]均高于对照组(t=5.178、7.984、3.065,均P<0.05)。结论中重度干眼症患者应用环孢素联合玻璃酸钠治疗的效果显著,可有效促进泪膜稳定性的恢复,缓解干眼症状。Objective To explore the clinical effectiveness of combining cyclosporine with sodium hyaluronate in the management of patients suffering from moderate to severe dry eye syndrome.Methods A total of 96 patients with moderate to severe dry eye admitted to Nanjing Tongren Hospital from March 2021 to March 2024 were selected.According to the treatment methods,these patients were divided into a control group(treated with sodium hyaluronate)and an observation group(treated with cyclosporine combined with sodium hyaluronate),with 48 in each group.The treatment effects,the corneal fluorescence staining(CFS)score,ocular surface disease index(OSDI)score,tear film breakup time(BUT),tear river height(TMH),tear secretion test(SIT)and conjunctival congestion before and after treatment were compared between the two groups.Results The effective rate of the observation group was 93.75%(45/48),which was higher than that of 79.17%(38/48)in the control(χ^(2)=4.360,P<0.05).The CFS,OSDI,and conjunctival congestion scores were all reduced after treatment in both groups,and the CFS((1.56±0.32)points vs(2.72±0.51)points),OSDI((20.91±4.36)points vs(20.91±4.36)points vs(27.28±4.42)points)and conjunctival congestion((1.12±0.25)points vs(1.39±0.32)points)were lower than those of the control group(t=3.348,7.108,4.607,all P<0.05).The BUT,TMH and SIT of the two groups of patients with moderate to severe dry eye after treatment were all higher than before treatment,and the BUT of the observation group((5.14±1.06)s vs(3.96±1.17)s),TMH((0.29±0.06)mm vs(0.20±0.05)mm)and SIT((5.46±1.62)mm·5 min^(-1) vs(4.53±1.34)mm·5 min^(-1))were higher than those in the control group(t=5.178,7.984,3.065,all P<0.05).Conclusion Treatment with cyclosporine combined with sodium hyaluronate is effective in patients with moderate to severe dry eye syndrome since it can effectively promote the recovery of tear film stability and relieve dry eye symptoms.
