莫沙必利联合柴胡疏肝散治疗老年胃食管反流(气郁痰阻证)患者的疗效

摘　　要：目的探讨莫沙必利联合柴胡疏肝散对中医症候为气郁痰阻证的老年胃食管反流病(GERD)的疗效。方法应用随机数字表法将2021年6月至2024年7月于北京市隆福医院接受治疗的104例GERD患者分为对照组与观察组,各52例。对照组采用莫沙必利治疗,观察组采用莫沙必利联合柴胡疏肝散治疗。比较两组患者临床疗效、中医症候积分、胃食管反流病量表(Gerd-Q)评分、血清胃肠激素水平以及不良反应情况。结果观察组治疗总有效率94.23%(49/52)高于对照组80.77%(42/52)(χ^(2)=4.308,P<0.05)。观察组治疗后的中医症候积分、Gerd-Q评分[分别为(6.82±1.76)分、(6.32±1.68)分]均低于对照组[分别为(10.13±2.09)分、(8.89±1.75)分](t=8.736、7.640,均P<0.05)。观察组治疗后的血管活性肠肽(VIP)水平[(17.37±3.20)pg·mL^(-1)]低于对照组[(24.92±3.34)pg·mL^(-1)](t=11.770,P<0.05),胃泌素(GAS)、胃动素(MTL)水平[分别为(68.42±5.54)pg·mL^(-1)、(256.16±24.84)pg·mL^(-1)]均高于对照组[分别为(56.21±5.76)pg·mL^(-1)、(214.58±23.61)pg·mL^(-1)](t=11.017、8.749,均P<0.05)。两组不良反应比较差异无统计学意义[11.54%(6/52)vs 15.38%(8/52),χ^(2)=0.330,P>0.05]。结论莫沙必利联合柴胡疏肝散治疗可提高老年气郁痰阻型GERD患者的疗效,有效调节胃肠激素水平,促进胃肠功能恢复,缓解相关症状体征,安全性良好。Objective To explore the efficacy of mosapride combined with bupleurum shugan powder in the treatment of gastroesophageal reflux(GERD)(called qi stagnation and phlegm obstruction syndrome in Traditional Chinese Medicine).Methods The random number table method was used to divide 104 GERD patients admitted to Beijing Longfu Hospital from June 2021 to July 2024 into the control group(treated with mosapride)and the observation group(treated with mosapride combined with bupleurum shugan powder),with 52 in each group.The clinical efficacy,TCM symptom scores,gastroesophageal reflux disease scale(Gerd-Q)scores,serum gastrointestinal hormone levels and incidence of adverse reactions were compared between the tgwo groups.Results The total effective rate was higher in the observation group than in the control(94.23%(49/52)vs 80.77%(42/52))(χ^(2)=4.308,P<0.05).The TCM symptom score and Gerd-Q score of the observation group after treatment were(6.82±1.76)points and(6.32±1.68)points and they were lower than those of the control((10.13±2.09)points,(8.89±1.75)points)(t=8.736,7.640,both P<0.05).The level of vasoactive intestinal peptide(VIP)of the observation group after treatment was(17.37±3.20)pg·mL^(-1),which was lower than that of(24.92±3.34)pg·mL^(-1) in the control(t=11.770,P<0.05),and the level of gastrin(GAS),motilin(MTL)((68.42±5.54)pg·m L^(-1),(256.16±24.84)pg·mL^(-1),respectively)were higher than those in the control((56.21±5.76)pg·mL^(-1),(214.58±23.61)pg·mL^(-1),respectively)(t=11.017,8.749,both P<0.05).There was no statistically significant difference in incidence of adverse reactions between the two groups(11.54%(6/52)vs 15.38%(8/52))(χ^(2)=0.330,P>0.05).Conclusion Mosapride combined with bupleurum shugan powder can significantly improve the efficacy of GERD in elderly patients with qi stagnation and phlegm obstruction since it can effectively regulate gastrointestinal hormone levels,promote gastrointestinal function recovery and relieve related symptoms and signs.Meanwhile,it is of good safety.
