中药调控相关炎症及凋亡通路治疗慢性萎缩性胃炎研究进展

摘　　要：细胞凋亡是一种基本的生物学现象,主要由炎症浸润转化而来,符合“炎—癌转化”模式。细胞凋亡不仅是一种特殊的细胞死亡类型,而且具有重要的生物学意义及复杂的分子生物学机制。细胞凋亡对于细胞的生存与发展及维持细胞稳态具有重要的意义。细胞凋亡对于慢性萎缩性胃炎(chronic atrophic gastritis,CAG)的意义主要在于调控其凋亡蛋白、减少炎症反应达到抵抗CAG的作用。研究发现中药复方及中药单体药物治疗CAG效果确切,中药可以调控相关信号通路治疗CAG,主要包括:细胞信号转导抑制因子3/Toll样受体4信号通路、磷脂酰肌醇-3激酶/蛋白激酶B信号通路、核转录因子-κB信号通路、Janus激酶2/信号转导和转录激活因子3信号通路、刺猬信号通路等,有效发挥调控炎症反应、细胞增殖与凋亡机制,促进胃黏膜恢复,阻断癌变。目前对这些研究进展缺乏系统性的整理与思考,因此本文将围绕这些信号通路和中药治疗CAG的作用机制的研究结果进行阐述,为中药治疗CAG提供相关的治疗思路。Apoptosis is a basic biological phenomenon,which is mainly transformed from inflammatory infiltration and conforms to the inflammation-cancer transformation mode.Apoptosis is not only a special type of cell death,but also has important biological significance and complex molecular biological mechanism.It is of great significance for cell survival and development to maintain cell homeostasis.The significance of apoptosis for chronic atrophic gastritis(CAG)is mainly to resist CAG by regulating its apoptotic protein and reducing inflammatory response.The study found that traditional Chinese medicine compound and monomer drugs have a prominent effect on CAG,and traditional Chinese medicine can regulate some related signaling pathways to treat CAG.It mainly includes:cell signal transduction inhibitor 3(SOCS3)/Toll-like receptor 4(TLR4)signaling pathway,phosphatidylinositol-3 kinase/protein kinase B(PI3K/Akt)signaling pathway,nuclear transcription factor-κB(NF-κB)signaling pathway,Janus kinase 2(JAK2)/signal transducer and activator of transcription 3(STAT3)signaling pathway,hedgehog and other signaling pathways,which can play a role in regulating inflammatory response,cell proliferation and apoptosis,promoting gastric mucosal recovery and blocking carcinogenesis.However,there is a lack of systematic collation and thinking.Therefore,these signaling pathways and the mechanism of action of traditional Chinese medicine in the treatment of CAG are elaborated on the research results in this field,so as to provide relevant treatment ideas for the treatment of CAG.
