颞下颌关节紊乱病采用稳定型咬合板联合半导体激光治疗的临床效果分析

摘　　要：目的 观察在颞下颌关节紊乱病患者中应用稳定型咬合板与半导体激光联合治疗的临床价值。方法 依据抽签法将64例颞下颌关节紊乱病患者分为参照组及试验组,每组32例。参照组开展稳定型咬合板治疗,试验组在参照组基础上增加半导体激光治疗。比较两组临床治疗效果,颞下颌关节紊乱指标[肌肉压痛指数(PI)、颞下颌关节功能障碍指数(DI)、Fricton颞下颌关节紊乱指数(CMI)],不同时间点张口度。结果 试验组临床治疗总有效率96.88%高于参照组的78.13%,具有统计学差异(P<0.05)。治疗后,两组PI、DI、CMI评分低于治疗前,且试验组PI(0.08±0.06)分、DI(0.20±0.08)分、CMI(0.12±0.04)分低于参照组的(0.12±0.02)、(0.27±0.09)、(0.17±0.02)分,具有统计学差异(P<0.05)。治疗1、3、6个月,试验组张口度分别为(30.24±1.32)、(33.24±1.38)、(35.54±3.28)mm,均大于参照组的(29.01±1.29)、(32.25±2.11)、(33.47±4.32)mm,具有统计学差异(P<0.05)。结论 将稳定型咬合板与半导体激光联合治疗应用在颞下颌关节紊乱病患者中具有显著作用,可改善张口度,值得应用。Objective To observe the clinical value of stabilization splint combined with semiconductor laser in the treatment of patients with temporomandibular joint disorders.Methods 64 patients with temporomandibular joint disorders were divided into the reference group and the experimental group by lottery method,with 32 cases in each group.The reference group was treated with stabilization splint,and the experimental group was treated with semicoductor laser based on the reference group.The clinical effect,indexes of temporomandibular joint disorder[ palpation index(PI),dysfunction index(DI),craniomandibular index(CMI)],and mouth opening at different time points were compared between the two groups.Results The total effective rate of 96.88% in the experimental group was higher than 78.13% in the reference group,and there was statistical difference(P<0.05).After treatment,PI,DI and CMI scores in both groups were lower than those before treatment;PI,DI and CMI scores were(0.08±0.06),(0.20±0.08) and(0.12±0.04) points in the experimental group,which were lower than(0.12±0.02),(0.27±0.09) and(0.17±0.02) points in the reference group;there was statistical difference(P<0.05).After 1,3 and 6 months of treatment,the mouth opening of the experimental group were(30.24±1.32),(33.24±1.38) and(35.54±3.28) mm,which were higher than(29.01±1.29),(32.25±2.11)and(33.47±4.32) mm of the reference group,and there was statistical difference(P<0.05).Conclusion Combined application of stabilization splint and semiconductor laser shows significant effect on patients with temporomandibular joint disorders,and can improve mouth opening and is worthy of application.
