慢性闭角型青光眼并白内障应用改良青光眼白内障联合手术治疗的效果分析

摘　　要：目的 分析慢性闭角型青光眼并白内障使用改良青光眼白内障联合手术治疗的临床效果。方法 选取慢性闭角型青光眼并白内障患者96例,按手术方式差异分为研究组与对照组,各48例。研究组以改良青光眼白内障联合手术进行治疗,对照组以常规青光眼白内障联合手术进行治疗。比较两组手术前后的眼压、视力水平、前房深度及术后30 d内的并发症发生情况。结果 两组术后3、30 d的眼压显著低于术前,且研究组术后3 d的眼压(23.22±1.56)mm Hg(1 mm Hg=0.133 kPa)、30 d的眼压(18.57±1.34)mm Hg显著低于对照组的(29.12±2.13)、(23.42±1.86)mm Hg,差异有统计学意义(P<0.05)。两组术后30 d的视力水平显著高于术前,且研究组术后30 d的视力水平(4.47±0.22)显著高于对照组的(4.16±0.13),差异有统计学意义(P<0.05)。两组术后30 d的前房深度显著大于术前,且研究组术后30 d的前房深度(2.89±0.17)mm显著大于对照组的(2.45±0.11)mm,差异有统计学意义(P<0.05)。研究组术后30 d内并发症发生率4.17%显著低于对照组的16.67%,差异有统计学意义(P<0.05)。结论 慢性闭角型青光眼并白内障使用改良青光眼白内障联合手术治疗可缓解患眼眼球内部压力,有助于恢复正常视力,且术后并发症少,其临床效果比常规手术治疗更为理想。Objective To analyze the clinical effect of modified glaucoma combined with cataract surgery for chronic angle-closure glaucoma and cataract.Methods 96 patients with chronic angle-closure glaucoma and cataract were selected and divided into study group and control group according to difference of surgical procedure,with 48 cases in each group.The study group was treated with modified glaucoma combined with cataract surgery,while the control group was treated with conventional glaucoma combined with cataract surgery.The intraocular pressure,visual acuity level,anterior chamber depth before and after surgery and complications within 30 d after surgery were compared between the two groups.Results The intraocular pressure at 3 and 30 d after surgery in both groups was significantly lower than that before surgery;the intraocular pressure were(23.22±1.56)and(18.57±1.34) mm Hg(1 mm Hg=0.133 kPa) at 3 and 30 d after surgery,which were significantly lower than(29.12±2.13) and(23.42±1.86) mm Hg in the control group;the difference was statistically significant(P<0.05).The visual acuity in both groups at 30 d after surgery was significantly higher than that before surgery,and the visual acuity of(4.47±0.22) at 30 d after surgery in the study group was significantly higher than(4.16±0.13) in the control group.The difference was statistically significant(P<0.05).The anterior chamber depth in both groups at 30 d after surgery was significantly higher than that before surgery,and the anterior chamber depth of(2.89±0.17) mm at 30 d after surgery in the study group was significantly higher than(2.45±0.11) mm in the control group.The difference was statistically significant(P<0.05).The incidence of complications within 30 d after surgery in the study group was 4.17%,which was significantly lower than 16.67% in the control group,and the difference was statistically significant(P<0.05).Conclusion Modified glaucoma combined with cataract surgery for patients with chronic angle-closure glaucoma and cataract can relieve the intr
