多西他赛联合顺铂同步放化疗治疗局部晚期非小细胞肺癌的疗效研究

摘　　要：目的 分析局部晚期非小细胞肺癌实施多西他赛+顺铂同步放化疗的效果。方法 以60例局部晚期非小细胞肺癌患者为调查样本,随机分为放疗组、同步组,各30例。放疗组行放疗治疗,同步组行多西他赛+顺铂同步放化疗治疗。比较两组治疗效果、肿瘤标志物[癌胚抗原(CEA)、鳞状细胞癌抗原(SCC)、糖类抗原199(CA199)]、不良反应发生情况、生活质量评分。结果 同步组缓解率为83.33%,放疗组为53.33%,同步组缓解率高于放疗组(P<0.05)。治疗后,两组CEA、SCC以及CA199水平均较治疗前降低,且同步组CEA(16.83±3.42)ng/ml、SCC(2.55±1.08)μg/L以及CA199(25.12±3.04)U/ml均低于放疗组的(23.61±3.39)ng/ml、(3.94±1.26)μg/L、(29.98±3.57)U/ml(P<0.05)。两组不良反应发生率比较无统计学差异(P>0.05)。同步组活力评分为(69.43±5.01)分,角色评分为(68.11±5.46)分,躯体评分为(65.23±5.89)分,社会评分为(67.27±5.33)分;放疗组活力评分为(61.89±5.27)分,角色评分为(60.95±5.13)分,躯体评分为(58.40±5.28)分,社会评分为(60.54±5.02)分,同步组活力、角色、躯体、社会评分均高于放疗组(P<0.05)。结论 多西他赛+顺铂同步放化疗治疗局部晚期非小细胞肺癌的缓解率高,可有效降低肿瘤标志物水平,改善生活质量,且不良反应少,有一定的临床推广意义。Objective To analyze the effect of concurrent chemoradiotherapy with docetaxel and cisplatin in the treatment of locally advanced non-small cell lung cancer.Methods 60 patients with locally advanced non-small cell lung cancer were selected as the investigation sample and randomly divided into radiotherapy group and synchronous group,each with 30 cases.The radiotherapy group received radiotherapy,and the synchronous group received concurrent chemoradiotherapy with docetaxel and cisplatin.The therapeutic effect,tumor markers[carcinoembryonic antigen(CEA),squamous cell carcinoma antigen(SCC),carbohydrate antigen 199(CA199)],adverse reactions and quality of life score were compared between the two groups.Results The remission rate was 83.33% in the synchronous group and 53.33% in the radiotherapy group;the remission rate of the synchronous group was higher than that of the radiotherapy group(P<0.05).After treatment,CEA,SCC and CA199 levels in both groups were lower than those before treatment;the synchronization group had CEA of(16.83±3.42) ng/ml,SCC of(2.55±1.08) μg/L and CA199 of(25.12±3.04) U/ml,which were lower than(23.61±3.39) ng/ml,(3.94±1.26) μg/L and(29.98±3.57) U/ml in the radiotherapy group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).In the synchronous group,the vitality score was(69.43±5.01) points,the role score was(68.11±5.46) points,the physical score was(65.23±5.89) points and the social score was(67.27±5.33) points;in the radiotherapy group,the vitality score was(61.89±5.27) points,the role score was(60.95±5.13) points,the physical score was(58.40±5.28) points and the social score was(60.54±5.02) points.The vitality,role,physical and social scores of the synchronous group were higher than those of the radiotherapy group(P<0.05).Conclusion Concurrent chemoradiotherapy with docetaxel and cisplatin in the treatment of locally advanced non-small cell lung cancer has a high remission rate,AND can effectively reduce the lev
