摘　　要：目的:分析抗血管内皮生长因子(VEGF)联合视网膜局部激光光凝治疗缺血型视网膜中央静脉阻塞继发黄斑水肿(CRVO-ME)的临床效果。方法:选取2021年9月—2023年10月句容市人民医院收治的96例缺血型CRVO-ME患者作为研究对象,以数字抽签方式将患者按1∶1随机分为对照组与试验组,各48例。对照组以雷珠单抗注射液行抗VEGF治疗,试验组在对照组基础上实施视网膜局部激光光凝治疗,比较两组疗效相关指标,房水相关因子及不良反应发生情况。结果:治疗后,两组最佳矫正视力(BCVA)、视网膜循环时间(RCT)、黄斑中心凹视网膜厚度(CMT)及房水白细胞介素-1β(IL-1β)、VEGF、白细胞介素-6(IL-6)浓度较治疗前降低,且试验组低于对照组,差异有统计学意义(P<0.05);试验组不良反应发生率(10.42%)与对照组(6.25%)相比,差异无统计学意义(P>0.05)。结论:抗VEGF联合视网膜局部激光光凝治疗缺血型CRVO-ME患者,可提高效果,促进相关指标恢复,下调房水VEGF及炎症因子表达,且安全性良好。Objective:To analyze the clinical effect of combining anti-vascular endothelial growth factor(VEGF)with retinal local laser photocoagulation in the treatment of macular edema secondary to ischemic central retinal vein occlusion(CRVO-ME).Method:A total of 96 patients with ischemic CRVO-ME who admitted to Jurong People's Hospital from September 2021 to October 2023 were selected as the research objects,the patients were randomly divided into the control group and the experimental group according to 1∶1 by digital lottery,with 48 cases in each group.The control group was treated with Ranibizumab Injection for anti-VEGF treatment,and the experimental group was treated with retinal local laser photocoagulation on the basis of the control group,the efficacy-related indicators,aqueous humor-related factors and adverse reactions were compared between two groups.Result:Compared with before treatment,the best corrected visual acuity(BCVA),retinal circulation time(RCT),central macular thickness(CMT)and the levels of interleukin-1β(IL-1β),VEGF and interleukin-6(IL-6)in aqueous humor in two groups were decreased after treatment,those in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the experimental group(10.42%)and the control group(6.25%)(P>0.05).Conclusion:Anti-VEGF combined with retinal local laser photocoagulation in the treatment of ischemic CRVO-ME patients can improve the effect,promote the recovery of related indicators,down-regulate the expression of VEGF and inflammatory factors in aqueous humor,and has good safety.
