腹腔镜治疗早期子宫内膜癌的疗效观察

摘　　要：目的观察腹腔镜治疗早期子宫内膜癌的疗效。方法选取84例早期子宫内膜癌患者,随机分为对照组和观察组,每组42例。对照组采用开腹手术治疗,观察组实施腹腔镜治疗。比较两组手术时间、术中出血量、盆腹腔引流量、并发症发生情况、尿管留置时间、住院时间、性功能评分。结果观察组手术时间(142.65±30.68)min短于对照组的(156.94±29.17)min,术中出血量(79.44±51.32)ml、盆腹腔引流量(508.62±103.81)ml明显少于对照组的(156.85±56.68)、(687.95±138.65)ml,差异显著(P<0.05)。观察组并发症发生率4.76%明显低于对照组的19.05%,差异显著(P<0.05)。观察组尿管留置时间(3.03±1.35)d、住院时间(7.31±1.41)d均明显短于对照组的(3.58±1.03)、(8.75±1.52)d,性功能评分(160.87±20.35)分明显高于对照组的(134.51±16.85)分,差异显著(P<0.05)。结论对早期子宫内膜癌患者实施腹腔镜治疗效果更佳,可显著减少手术创伤,避免并发症发生,实现术后快速恢复,降低对患者性功能的影响,有较高应用价值。Objective To observe the curative effect of laparoscopy on early-stage endometrial carcinoma.Methods A total of 84 patients with early-stage endometrial carcinoma were randomly divided into control group and observation group,with 42 cases in each group.The control group was treated with open surgery,and the observation group was treated with laparoscopy.The operation time,intraoperative blood loss,peritoneal drainage,complications,duration of indwelling urinary catheterization,length of hospital stay and sexual function score were compared between the two groups.Results The observation group had shorter operation time of(142.65±30.68)min than(156.94±29.17)min in the control group;the intraoperative blood loss and peritoneal drainage were(79.44±51.32)and(508.62±103.81)ml in the observation group,which were significantly less than(156.85±56.68)and(687.95±138.65)ml in the control group;the difference was significant(P<0.05).The complication rate of 4.76%in the observation group was significantly lower than 19.05%in the control group(P<0.05).The duration of indwelling urinary catheterization and length of hospital stay were(3.03±1.35)and(7.31±1.41)d in the observation group,which were significantly shorter than(3.58±1.03)and(8.75±1.52)d in the control group;the observation group had significantly higher sexual function score of(160.87±20.35)points than(134.51±16.85)points in the control group;the difference was significant(P<0.05).Conclusion Laparoscopy has better effect on patients with early-stage endometrial carcinoma,and can significantly reduce surgical trauma,avoid complications,achieve rapid postoperative recovery,and reduce the impact on patients'sexual function,which is of high application value.
