首发精神分裂症患者颅脑MRI影像学与临床症状的相关性分析

摘  要:目的:探析首发精神分裂症患者颅脑MRI影像学与临床症状的相关性。方法:选取82例首发精神分裂症患者作为观察组,另按1∶1选同期82例健康者作为对照组。两组均行颅脑MRI检查,比较两组外侧裂宽度、大脑中线至穹隆脚下方距离(FMD)、胼胝体侧与穹隆脚下方水平距离(CFLD)、胼胝体侧与穹隆脚下方垂直距离(CFPD)、紧贴胼胝体穹隆脚直径(FD1)、穹隆脚中段直径(FD2)、脑室面积(VA)等MRI影像学指标差异;采用阳性和阴性症状量表(PANSS)及蒙特利尔认知评估(MoCA)量表评估观察组患者临床症状,Pearson法分析上述影像学指标与临床症状的关系。结果:两组左侧外侧裂宽度、CFLD、CFPD及VA均高于右侧,FD2均低于右侧(P<0.05),与对照组相比,观察组双侧外侧裂宽度、CFLD、CFPD更长,FD2更短,VA更高(P<0.05);观察组患者PANSS总分为(86.11±8.24)分,阳性症状评分为(28.95±6.17)分,阴性症状评分为(21.57±5.36)分,一般精神病理学评分为(35.59±11.01)分,MoCA评分为(14.57±3.00)分。Pearson相关性分析显示,首发精神分裂症患者双侧外侧裂宽度与阳性症状评分呈正相关(r=0.266、0.264),FD2与阳性症状评分呈负相关(r=-0.243、-0.337),双侧外侧裂宽度与MoCA评分呈负相关(r=-0.615、-0.458)(P均<0.05)。结论:首发精神分裂症患者大脑左右不对称,双侧外侧裂宽度、CFLD、CFPD、VA均高于正常人,FD2均低于正常人,且其外侧裂宽度、FD2分别与临床症状存在关联性。Objective:To explore the correlation between brain MRI parameters and clinical symptoms in patients with first-episode schizophrenia.Methods:A total of 82 patients with first-episode schizophrenia were selected as the observation group.Meanwhile,82 healthy individuals were selected as the control group in a 1∶1 ratio.Both groups underwent brain MRI examination.The differences in MRI parameters such as width of lateral fissure,distance from brain midline to lower crus of the fornix(FMD),horizontal distance from lateral side of corpus callosum to lower crus of the fornix(CFLD),vertical distance from lateral side of corpus callosum to lower crus of the fornix(CFPD),diameter of the crus of the fornix adjacent to corpus callosum(FD1),diameter of the middle part of the crus of the fornix(FD2)and ventricular area(VA)between the two groups were analyzed.The positive and negative syndrome scale(PANSS)and Montreal cognitive assessment(MoCA)were used to evaluate clinical symptoms of the observation group.Pearson correlation analysis was used to analyze the relationship between above-mentioned imaging indicators and clinical symptoms.Results:In both groups,the width of lateral fissure,CFLD,CFPD and VA on the left were greater than those on the right,and FD2 was smaller than that on the right(P<0.05).Compared with the control group,the observation group had larger lateral fissure,CFLD and CFPD,shorter FD2 and larger VA(P<0.05).The total PANSS score,positive symptom score,negative symptom score,general psychopathology score and MoCA score of the observation group were(86.11±8.24),(28.95±6.17),(21.57±5.36),(35.59±11.01)and(14.57±3.00).Pearson correlation analysis found that the width of bilateral lateral fissures was positively correlated with positive symptom scores in patients with firstepisode schizophrenia(r=0.266,0.264).FD2 was negatively correlated with positive symptom scores(r=-0.243,-0.337)(all P<0.05).Conclusion:Patients with first-episode schizophrenia have brain lateralization.Compared to normal individuals,t
