应用锥形束CT重建分析氟牙症患者颞下颌关节骨结构特征

摘  要:目的应用锥形束CT测量比较氟牙症和非氟牙症患者颞下颌关节骨结构特征的差异。方法选取我院就诊患者60例,根据氟牙症标准分为氟牙症组和对照组,所有患者均行锥形束CT扫描,采用NNT软件测量颞下颌关节骨结构相关参数,应用Mimics软件测量髁突骨密度,进行统计学分析。结果氟牙症组颞下颌关节骨质变化发生率明显高于对照组(χ^(2)=7.5,P=0.006)。氟牙症组颞下颌关节中间隙、外间隙、上间隙和后间隙较对照组减小(t=2.330,P=0.023;t=2.195,P=0.032;t=2.249,P=0.028;t=2.528,P=0.014)。氟牙症组髁突最前点骨密度降低(t=2.477,P=0.016),最顶点骨密度升高(t=-2.227,P=0.030)。结论氟牙症患者颞下颌关节髁突骨质和关节间隙改变,提示氟中毒不仅导致氟牙症,还可能是导致颞下颌关节病变的一个危险因素。Objective Application of cone beam CT measurement to observe the differences in the bone structure characteristics of the temporomandibular joint between patients with fluorosis and those without fluorosis.Methods Sixty patients who visited our hospital were selected and divided into a fluorosis group and a control group based on whether they had fluorosis.All patients underwent cone-beam computed tomography scans,and the temporomandibular joint bone structure related parameters were measured using NNT software.The condylar bonedensity was measured using materialise’s interactive medical imagecontrol system software,and statistical analysis was performed.Results The incidence of temporomandibular joint bone changes in the fluorosis group was significantly higher than that in the control group(χ^(2)=7.5,P=0.006).The center space,lateral space,superior space and posterior space in the fluorosis group were reduced compared to the control group(t=2.330,P=0.023;t=2.195,P=0.032;t=2.249,P=0.028;t=2.528,P=0.014).The bone density at the anterior point of the condyle in the fluorosis group was decreased(t=2.477,P=0.016),while the bone density at the apex was increased(t=-2.227,P=0.030).Conclusion The changes in the condyle bone and joint space of patients with fluorosis suggest that fluorosis not only leads to fluorosis,but also may be a risk factor for temporomandibular joint lesions.
