免疫球蛋白静滴方案对自身免疫性脑炎患者疗效及机体免疫、炎症反应水平的影响

摘　　要：目的:评价免疫球蛋白静滴方案在自身免疫性脑炎患者治疗中的应用价值。方法:随机选择2022年1月至2023年12月间我院收治的自身免疫性脑炎患者80例,按照治疗方式不同分为对照组与实验组(n=40),分别静脉滴注甲泼尼龙琥珀酸钠1000mg以及在此基础上静脉滴注免疫球蛋白2g·kg^(-1)治疗,2w后评估治疗效果,采用流式细胞术测定机体免疫能力,并通过酶联免疫吸附检测炎症反应水平。结果:实验组治疗有效率相比对照组更高,组间数据差异显著(P<0.05);实验组临床症状缓解时间、精神状态好转时间、住院时长以及炎症反应相关指标数值相较于对照组更低,组间数据差异显著(P<0.05);实验组治疗后CD4细胞阳性(CD4^(+))、CD4细胞阳性与CD8细胞阳性比值(CD4^(+)/CD8^(+))相比对照组更高,而CD8细胞阳性(CD8^(+))相比对照组更低,组间数据差异显著(P<0.05)。结论:为自身免疫性脑炎患者提供免疫球蛋白静滴方案,可以加速患者痊愈,改善机体免疫功能以及炎症反应水平。Objective:To evaluate the application value of immunoglobulin intravenous drip in treating autoimmune encephalitis patients.Methods:Eighty patients with autoimmune encephalitis admitted to our hospital from January 2022 to December 2023 were randomly selected and divided into a control group and an experimental group(n=40)according to different treatment methods.They were treated with an intravenous infusion of methylprednisolone sodium succinate 1000 mg and an intravenous infusion of immunoglobulin 2 g·kg^(-1) on this basis,respectively.After 2 weeks,the therapeutic effect was evaluated,and the body's immune ability was measured by flow cytometry.The level of inflammatory response was detected by enzyme-linked immunosorbent assay.Results:The treatment efficacy rate in the experimental group was higher than that in the control group,and the difference between the groups was statistically significant(P<0.05).The clinical symptom relief time,mental state improvement time,length of hospital stay,and inflammation-related indicators in the experimental group were lower than those in the control group,and the differences between the groups were significant(P<0.05).After treatment,the CD4 cell positive(CD4^(+))and the ratio of CD4 cell positive to CD8 cell positive(CD4^(+)/CD8^(+))in the experimental group were higher than those in the control group,while the CD8 cell positive(CD8^(+))was lower than that in the control group.The data differences between the groups were significant(P<0.05).Conclusion:Providing intravenous immunoglobulin infusion protocols for patients with autoimmune encephalitis can accelerate their recovery,improve their immune function,and reduce their inflammatory response levels.
