维奈克拉联合阿糖胞苷治疗对老年急性髓系白血病临床疗效、免疫功能及不良反应的影响

摘　　要：目的:探讨维奈克拉联合阿糖胞苷治疗对老年急性髓系白血病(Acute Myeloid Leukemia,AML)临床疗效、免疫功能及不良反应的影响。方法:回顾性收集2021年1月至2024年1月期间本院收治的72例老年AML患者的临床资料。依据治疗方式的不同,将患者分为对照组(采用阿糖胞苷治疗,35例)和试验组(采用阿糖胞苷+维奈克拉片治疗,37例)。分析对比两组的临床疗效、免疫功能(CD8^(+)、CD4^(+)和CD4^(+)/CD8^(+)比值)、血液指标值(血红蛋白水平、白细胞计数、血小板计数和骨髓原始细胞比例)及不良反应。结果:试验组的治疗总疗效率明显高于对照组(P<0.05)。试验组的CD4^(+)、CD4^(+)/CD8^(+)水平比对照组显著提高,CD8^(+)水平比对照组显著降低(P>0.05)。试验组血红蛋白水平、白细胞计数、血小板计数均比对照组显著提高,骨髓原始细胞比例比对照组显著降低(P<0.05)。两组的不良反应发生率无显著差异。结论:采用维奈克拉联合阿糖胞苷治疗老年AML,能提高治疗效果,改善患者免疫功能和血液指标,且用药安全性较高。Objective:To investigate the clinical efficacy,immune function and adverse reactions of Venetoclax combined with cytarabine in the treatment of senile Acute Myeloid Leukemia(AML).Methods:The clinical data of 72 elderly patients with AML admitted to our hospital from January 2021 to January 2024 were retrospectively collected.According to different treatment methods,the patients were divided into control group(treated with cytarabine,35 cases)and experimental group(treated with cytarabine and Venetoclax tablets,37 cases).The clinical efficacy,immune function(CD8^(+),CD4^(+)and CD4^(+)/CD8^(+)ratio),blood index value(hemoglobin level,white blood cell count,platelet count and bone marrow original cell ratio)and adverse reactions of the two groups were analyzed and compared.Results:The total therapeutic effect rate of experimental group was significantly higher than that of control group(P<0.05).The levels of CD4^(+)and CD4^(+)/CD8^(+)in experimental group were significantly higher than those in control group,and the levels of CD8^(+)were significantly lower than those in control group(P>0.05).The hemoglobin level,white blood cell count and platelet count of experimental group were significantly increased compared with control group,and the proportion of bone marrow original cells was significantly decreased compared with control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion:The combination of Venetoclax and cytarabine in the treatment of elderly AML can improve the therapeutic effect,immune function and blood indexes of patients,with high safety.
