摘 要:目的:探索融合情景模拟及视频反馈的混合教学法在儿科实习医生医患沟通能力培养中的作用。方法:选取2020年1月~2022年12月在弋矶山医院实习的60名临床专业学生,问卷调查学生对医患关系及医患沟通的整体认知情况,按随机数字表法分为对照组和研究组各30名。对照组采用传统教学法,研究组采用融合情景模拟及视频反馈的混合教学法,出科前进行考核:包括理论、技能考核、教学认可度及医患沟通能力评估。结果:研究组理论、技能考核成绩及医患沟通技能评价量表得分均高于对照组[(86.63±4.33)分vs.(81.83±6.87)分;(83.30±4.99)分vs.(76.20±6.07)分;(20.10±2.14)分vs.(17.37±2.82)分,P<0.01];研究组医患沟通能力评分良好率[63.33%(19/30)vs.16.67%(5/30)]及对带教方式在提高医患沟通能力[86.67%(26/30)vs.46.67%(14/30)]、提高临床思维能力[90.00%(27/30)vs.60.00%(18/30)]、增强职业信心[83.33%(25/30)vs.50.00%(15/30)]的认可率均高于对照组(P<0.05)。结论:融合情景模拟及视频反馈的混合教学法在儿科实习医生带教中的应用效果较好,不仅提高医患沟通能力,还提升临床理论、技能掌握程度,获得实习医生认可。Objective:To observe the effect of video feedback plus scenario simulation teaching mode in cultivating doctor-patient communication ability of pediatric interns.Methods:Sixty students majoring in clinical medicine who interned at our hospital were included from January 2020 to December 2022,and were randomized to control group and experimental group(n=30 for each group)by random number table.A questionnaire survey was conducted to assess the overall understanding of doctor-patient relationships and communication.The control group underwent traditional teaching,and the experimental group was given video feedback plus scenario simulation teaching mode.Before completion of the internship in our department,the students underwent evaluations based on theoretical examination,skill assessment,teaching satisfaction survey,and doctor-patient communication ability test.Results:The experimental group had significantly higher average scores in theoretical examination,skill assessment and SEGUE scale for doctor-patient communication ability than the control group[(86.63±4.33)vs.(81.83±6.87),(83.30±4.99)vs.(76.20±6.07),(20.10±2.14)vs.(17.37±2.82),P<0.01].The experimental group showed significantly higher rates in good doctor-patient communication ability(63.33%,19/30 vs.16.67%,5/30),as well as higher approval rates for the teaching method in improving doctor-patient communication ability(86.67%,26/30 vs.46.67%,14/30),enhanced clinical thinking ability(90.00%,27/30 vs.60.00%,18/30),and boosted professional confidence(83.33%,25/30 vs.50.00%,15/30)compared to the control group(P<0.05).Conclusion:Video feedback plus scenario simulation teaching mode demonstrates effective results in the training of pediatric interns,leading to improved doctor-patient communication ability,enhanced clinical theory understanding and skill mastery,and higher satisfaction with the teaching among interns.
