益生菌及盐酸多奈哌齐治疗阿尔茨海默病临床观察

摘  要:目的探讨益生菌及盐酸多奈哌齐治疗对阿尔茨海默病(AD)患者认知功能的影响,并分析认知功能与血清NOD样受体家族蛋白3(NLRP3)、可溶性CD22(sCD22)、钙黏蛋白13(CDH13)基因相关性。方法选取AD患者280例,随机数字表法分为对照组(基础治疗+盐酸多奈哌齐治疗,140例)和观察组(基础治疗+益生菌及盐酸多奈哌齐治疗,140例),比较2组患者认知功能及血清NLRP3、sCD22、CDH13 mRNA,并采用Pearson分析认知功能与血清NLRP3、sCD22、CDH13 mRNA的相关性。结果与治疗前比较,治疗后6个月2组患者简易精神状态检查量表(MMSE)评分均升高,阿尔茨海默病评估量表认知次量表(ADAS-Cog)评分降低,且与对照组比较,观察组治疗后6个月MMSE评分更高,ADAS-Cog评分更低,差异均有统计学意义(P<0.05)。与治疗前比较,2组患者治疗后6个月血清NLRP3、sCD22、CDH13 mRNA降低,且与对照组比较,观察组更低,差异有统计学意义(P<0.05)。经Pearson分析,治疗前、治疗后6个月MMSE评分与血清NLRP3、sCD22、CDH13 mRNA呈负相关(P<0.05),治疗前、治疗后6个月ADAS-Cog评分与血清NLRP3、sCD22、CDH13 mRNA呈正相关(P<0.05)。结论益生菌及盐酸多奈哌齐治疗AD患者,可改善认知功能,降低血清NLRP3、sCD22、CDH13 mRNA,AD患者认知功能与血清NLRP3、sCD22、CDH13 mRNA具有关联性。Objective To explore the impact of probiotics and donepezil hydrochloride treatment on the cognitive function of patients with Alzheimer’s disease(AD),and to analyze the correlation between cognitive function and serum NOD-like receptor family protein 3(NLRP3),soluble CD22(sCD22),and cadherin 13 gene(CDH13).Methods A total of 280 AD patients were selected and divided into a control group(basic treatment+donepezil hydrochloride treatment,140 cases)and an observation group(basic treatment+probiotics and donepezil hydrochloride treatment,140 cases)by the random number table method.The cognitive function and serum NLRP3,sCD22,and CDH13 mRNA of the two groups of patients were compared,and Pearson analysis was used to analyze the correlation between cognitive function and serum NLRP3,sCD22,and CDH13 mRNA.Results Compared with before treatment,the Mini-Mental State Examination(MMSE)scores of the two groups of patients increased and the scores of the Alzheimer’s Disease Assessment Scale-Cognitive Subscale(ADAS-Cog)decreased 6 months after treatment.Moreover,compared with the control group,the MMSE scores of the observation group were higher and the ADAS-Cog scores were lower 6 months after treatment,and the differences were statistically significant(P<0.05).Compared with before treatment,the serum NLRP3,sCD22,and CDH13 mRNA of the two groups of patients decreased 6 months after treatment,and the levels in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).Pearson analysis showed that the MMSE scores before treatment and 6 months after treatment were negatively correlated with serum NLRP3,sCD22,and CDH13 mRNA(P<0.05),and the ADAS-Cog scores before treatment and 6 months after treatment were positively correlated with serum NLRP3,sCD22,and CDH13 mRNA(P<0.05).Conclusion The treatment of AD patients with probiotics and donepezil hydrochloride can improve cognitive function and reduce serum NLRP3,sCD22,and CDH13 mRNA.There is a correlation between the cognit
