基于数据挖掘探析针灸治疗恶性肠梗阻的选穴规律

摘　　要：目的:通过数据挖掘技术深入探究针灸治疗恶性肠梗阻的选穴规律。方法:检索中国知网、万方数据库、维普中文科技期刊三大中文数据库自建库至2024年9月1日收录的有关针灸治疗恶性肠梗阻的临床研究文献,按照拟定的纳入标准及排除标准筛选后运用Excel软件对纳入文献数据进行计量分析,再使用SPSS软件进行关联规则分析和聚类分析。结果:最终纳入文献56篇,涉及腧穴39个,应用总频次为296,使用频次最高的腧穴为足三里穴、上巨虚穴、天枢穴、中脘穴、下巨虚穴;足阳明胃经、任脉及手阳明大肠经为主要选穴归经;下肢、胸腹部及上肢为腧穴主要分布部位;采用最多的特定穴类型为下合穴;通过关联规则分析得到13条强关联规则,最高支持度组合为足三里穴–上巨虚穴;聚类分析得出4类有效腧穴配伍。结论:针灸治疗恶性肠梗阻以补益脾胃以促进其功能恢复为主,治标兼固本,体现了“经脉所在,主治所在”及“经脉所过,主治所及”的选穴思路。本研究总结分析了针灸治疗恶性肠梗阻的选穴应用规律,以期为后续临床治疗恶性肠梗阻提供相应数据参考。Objective:To explore the rule of acupuncture point selection in treating malignant bowel obstruction based on data mining technology.Methods:The clinical research literature on acupuncture in the treatment of malignant bowel obstruction included in the CNKI,Wanfang and VIP databases from the establishment of the database to September 1,2024 was searched.After screening according to the inclusion criteria and exclusion criteria,the data of the included literature was measured and analyzed using Excel software.Association rule analysis and cluster analysis were performed using SPSS software.Results:A total of 56 articles were included,involving 39 acupoints,and the total frequency of application was 296.Among them,the most frequently used acupoints of all acupoints were Zusanli(ST36),Shangjuxu(ST37),Tianshu(ST25),Zhongwan(RN12),and Xiajuxu(ST39).When selecting acupoints,stomach channel of foot-Yangming(阳明),Conception Vessel and large intestine channel of hand-Yangming were the main meridians.The lower limbs,chest,abdomen and upper limbs were the main distribution parts of acupoints.The most commonly used specific acupoint type was the Xiahe points(下合穴).The analysis of association rules yielded 13 strong association rules,and the highest support combination was Zusanli acupoint-Shangjuxu acupoint.Cluster analysis showed that four types of effective acupoints were compatible.Conclusion:Acupuncture treatment of malignant bowel obstruction can promote the recovery of intestinal function by invigorating spleen and stomach,treating disease by looking into both its root cause and symptoms.It embodies the idea of acupoint selection of“Where are the meridians located,and where are the main acupoints selected”and“Where the meridians pass,the main treatment reaches”.This study summarizes and analyzes the application rules of acupoint selection in acupuncture and moxibustion for the treatment of malignant bowel obstruction,aiming to provide corresponding data references for subsequent clinical treatment of ma
