基于风药开玄理论探讨脑转移瘤的治疗思路

摘　　要：“玄府”一词首见于《黄帝内经》,由刘完素完善其内涵。玄府为遍布人体各处的一种微细的孔窍及其通道结构,具有流通气液、渗灌气血、调理阴阳的作用,是气机升降之门户、气液流通之通道。五脏六腑皆有玄府,脑亦然,脑玄府开阖功能正常的中心环节为脑内气、血、津液正常流通,阴阳平衡。脑玄府以通为顺,以闭为逆,玄府闭塞,百病由生。脑转移瘤的发生是素体正虚与气滞、痰浊、血瘀等邪毒侵袭共同作用导致脏腑气血阴阳失调,阴阳不相顺接,内风自生所致,还与脑玄府闭塞密切相关。脑玄府具有“因郁而闭、因虚而滞”的特点,对于脑转移瘤患者而言,气血虚弱是玄府衰竭闭塞的基础,气机逆乱、痰浊阻滞、瘀血停滞是玄府邪实闭塞的关键。在治疗时,应借助风药之特性,以开通脑玄府为原则,使玄府气机畅达,津液输布正常。风药是指具有“升、发、散、窜、通”特性的一类中药,具有升阳调气、引经报使、开发郁结的作用,利用其宣通走窜之性可疏通痰浊、瘀血而使津液畅达、脉道通畅。其疏散之性可使补虚之品补益而不滋腻,行气而不耗气,使脑玄府功能恢复。文章基于玄府理论探讨风药在治疗脑部转移性肿瘤方面的潜力,以期为中医药治疗脑转移瘤提供新的思路,丰富脑转移瘤的治疗方案。The word Xuanfu(玄府)was first found in Huangdi Neijing(《黄帝内经》),and its connotation was perfected by LIU Wansu.Xuanfu is a kind of fine orifice and channel structure spread all over the human body.It has the function of circulating Qi(气)and liquid,permeating Qi and blood,regulating Yin(阴)and Yang(阳),and is the gateway of Qi circulation,and the channel of Qi and liquid circulation.The five viscera and six Fu-organs(腑)have Xuanfu,and so does the brain.The central link of the normal opening and closing function of the brain Xuanfu is the normal circulation of Qi,blood,and body fluid in the brain,that is the balance of Yin and Yang.When the brain Xuanfu is unobstructed,the body's various functions can be played normally.When the brain Xuanfu is blocked,the circulation of Qi,blood,and body fluid is blocked,leading to various diseases.The occurrence of brain metastases is caused by the combination of deficiency of the body and the invasion of Qi stagnation,phlegm turbidities,blood stasis and other evil toxins,which leads to the imbalances of Qi and blood,and Yin and Yang of the Zang-Fu(脏腑)organs,the incoherence of Yin and Yang,and the spontaneous generation of internal wind.It is also closely related to the blockage of brain Xuanfu,which is characterized by“closure due to depression and stagnation due to deficiency”.For patients with brain metastases,weakness of Qi and blood is the basis for the failure and blockage of Xuanfu.The key to the obstruction of Xuanfu is the disturbance of Qi,blockage of phlegm turbidity,and stagnation of blood stasis.In the treatment,we should make use of the characteristics of wind-medicine and take the principle of opening the brain Xuanfu to make Xuanfu Qi flow smoothly and body fluid distribution normal.Wind-medicine refers to a kind of Chinese medicine with the characteristics of“rising,inducing,dispersing,channeling,dredging”.It has the function of raising Yang to regulate Qi,leading the medicine into the meridians,and dispersing stasis.By using its p
