口服祛泡剂结合内镜下西甲硅油稀释液冲洗在无痛电子胃镜检查中的应用效果评估

摘　　要：目的探讨口服袪泡剂结合内镜下西甲硅油稀释液冲洗在无痛电子胃镜检查中的应用效果和方法。方法选取门诊行无痛电子胃镜检查达到内镜下视野清晰度评分标准≥2分者528例次,将其按随机数字表法均分为A、B、C三组,每组176例次,所有患者均在检查前口服袪泡剂。A组为西甲硅油稀释液冲洗组,B组为纯净水冲洗组,C组为负压吸引组,观察比较三组患者内镜下视野清晰度、微小病灶检出率、检查时长、平均冲洗次数和平均冲洗量及医师满意度。结果A组内镜下视野清晰度显著高于B组、C组,差异具有统计学意义(P<0.05);A组微小病灶检出情况优于C组;A组平均冲洗次数(2±1)次和平均冲洗量(38±6)ml少于B组(5±3)次和(105±25)ml,差异具有统计学意义(P<0.05);A组内镜检查时长(13±1.2)min少于B组(18±2.3)min、C组(17±1.9)min,差异具有统计学意义(P<0.05);A组医师满意度(97.7%)优于B组(73.9%)、C组(63.1%),差异具有统计学意义(P<0.05)。结论口服祛泡剂结合内镜下西甲硅油稀释液冲洗可明显减少胃黏膜表面附着的泡沫和黏液,有效提高内镜下视野清晰度和微小病灶检出率,缩短检查时长,降低麻醉风险,提高了医师的满意度,获得了良好的效果。Objective To explore the application effect of oral defoaming and method of endoscopic rinsing with of simethicone diluent in painless electronic gastroscopy examination.Methods A total of 528 patients who underwent painless gastroscopy examinations in the outpatient department and achieved a score of≥2 points for endoscopic visual field clarity were randomly divided into 3 groups using a random number table method,and each group included 176 patients.All patients were treated with oral defoaming before examination.Group A was treated with simethicone diluent,Group B was treated with pure water,and Group C was treated with negative pressure suction.The endoscopic visual field clarity,detection rate of small lesions,examination duration,average number of washes,average amount of washes,and physician satisfaction were observed and compared among the three groups of patients.Results The visual field of Group A was clearer than Group B and Group C under endoscopy,and the difference was statistically significant(P<0.05).The detection of small lesions of Group A was better than that in Group C.As well,the mean flushing times(2±1)times and mean flushing volume(38±6)ml in Group A was less compare to Group B[(5±3)times,(105±25)ml],respectively,and the differences were statistically significant(P<0.05).The duration of endoscopy in Group A(13±1.2)min was shorter than Group B(18±2.3)min and Group C(17±1.9)min,and the difference was statistically significant(P<0.05).The physician satisfaction in Group A(97.7%)was better than that in Group B(73.9%)and Group C(63.1%),respectively,and the difference was statistically significant(P<0.05).Conclusion Oral defoaming agent combined with endoscopic rinsing of simethicone diluent can significantly reduce the attached foam and mucus on the surface of gastric mucosa,effectively improve the visual field clarity under endoscopy and the detection rate of small lesions,shorten the examination duration,reduce the risk of anesthesia,improve the satisfaction of physicians,and obtain go
