精神分裂症患者动机缺乏对认知功能的影响及其相关性分析

摘　　要：目的探讨精神分裂症患者动机缺乏对认知功能的影响,并分析其相关性。方法纳入2022年1月至2024年5月收治的60例精神分裂症患者为研究对象。收集患者一般资料,入院时应用阳性症状评估量表(SAPS)、阴性症状评分(SANS)、功能大体评定量表(GAF)、加州语言学习测试(CVLT)、内在动机量表(IMI-SR)分别评定患者阳性症状、阴性症状、社会功能、认知水平及内在动机水平。比较不同临床特征精神分裂症患者IMI-SR总分情况,分析上述评分与IMI-SR评分之间相关性。结果精神分裂症患者IMI-SR总分、SAPS评分、SANS评分、GAF评分、CVLT总分分别为(94.75±6.34)分、(4.40±0.65)分、(85.41±6.62)分、(55.19±7.81)分、(51.44±6.34)分。不同SAPS、SANS、GAF、CVLT评分精神分裂症患者的IMI-SR总分比较,差异均有统计学意义(t分别=7.66、8.93、9.39、6.14,P均<0.05)。相关性分析显示,IMI-SR评分与SAPS、SANS呈负相关(r分别=-0.42、-0.48,P均<0.05),与GAF、CVLT评分呈正相关(r分别=0.52、0.54,P均<0.05)。多元线性回归分析结果显示,SAPS评分≥4分、SANS评分≥85分、GAF评分<55分和CVLT评分<51分均为影响IMI-SR评分的影响因素(t分别=-4.46、-3.26、2.43、5.82,P均<0.05)。结论精神分裂症患者存在内部动机和认知功能损害,且与患者阳性、阴性症状、社会功能和认知水平具一定相关。Objective To explore the effects of motivation deficiency on cognitive function and analyze their correlation in schizophrenia patients.Methods A total of 60 patients with schizophrenia admitted to the hospital were enrolled as the research objects between January 2022 and May 2024.The general data of patients were collected.At admission,negative symptoms,positive symptoms,social function,cognitive level and intrinsic motivation were evaluated by scale for the assessment of positive symptoms(SAPS),scale for the assessment of negative symptoms(SANS),global assessment of function(GAF),california verbal learning test(CVLT)and intrinsic motivation inventory for schizophrenia research(IMI-SR),respectively.The total score of IMI-SR in schizophrenia patients with different clinical characteristics was compared,and the correlation between the above scores and IMI-SR score was analyzed.Results The total score of IMI-SR,SAPS score,total score of SANS,GAF score and total score of CVLT were(94.75±6.34)points,(4.40±0.65)points,(85.41±6.62)points,(55.19±7.81)points and(51.44±6.34)points,respectively.The differences in total score of IMI-SR among schizophrenia patients with different scores of SAPS,SANS,GAF and CVLT were statistically significant(t=7.66,8.93,9.39,6.14,P<0.05).Correlation analysis showed that IMI-SR score was negatively correlated with SAPS and SANS scores(r=-0.42,-0.48,P<0.05),while positively correlated with GAF and CVLT scores(r=0.52,0.54,P<0.05).The results of multiple linear regression analysis showed that SAPS score≥4 points,SANS score≥85 points,GAF score<55 points and CVLT score<51 points were all influencing factors of IMI-SR score(t=-4.46,-3.26,2.43,5.82,P<0.05).Conclusion There is impairment of intrinsic motivation and cognitive function in schizophrenia patients,which has certain correlation with positive symptoms,negative symptoms,social function and cognitive level.
