摘　　要：针对当下临床中医患沟通的实际问题,助力于叙事医学的本土化实践,介绍了4个中医叙事医学临床实践路径配套的工具清单:倾听要素清单、标本缓急清单、制定方案三方清单、标本缓急记录(患者版),以青光眼诊疗为例,从使用说明和应用示范2个层面对每个清单的应用场景、使用方法、方式、注意事项等进行逐一阐述,以帮助临床工作者在与患者沟通中更加高效地收集患方的疾病故事与循证证据,提升医疗质量。此外,提供了一种以患者为中心进行身心状况记录的辅助工具,有利于提高患者参与度,改善医患沟通。In response to the practical challenges of communication between clinicians and patients in Traditional Chinese Medicine(TCM),and to support the localization of narrative medicine practices,this paper introduces four clinical practice pathway checklists for TCM narrative medicine:the listening elements checklist,the root-branch-priority checklist,the tripartite plan-making checklist,and the root-branch-priority record(patient version).For clear explanation of four checklists,this paper uses glaucoma diagnosis and treatment as an example to elaborate the application scenarios,usage methods,procedures,and precautions for each checklist from two aspects:instructions for use and application demonstration.This aims to assist clinicians in more efficiently collecting patient stories and evidence-based data during communication,thereby improving the quality of care.Additionally,the root-branch-priority record(patient version) is the record in a patient-centered manner for aiding patients to record their physical and mental conditions,which helps to enhance patient engagement and improve doctor-patient communication.
