外折重睑矫正为内折重睑的手术策略:低位设计、方向控制缝合和脂肪转移

摘　　要：目的探究一种外折重睑修复为内折重睑的手术策略,以达到更好的重睑美学效果。方法回顾性分析自2022年1月至2024年1月就诊于广州颜所医疗美容门诊部外折重睑102例患者的临床资料,分为3组:先天性外折重睑组(A组)、既往术后有内眦赘皮的外折重睑组(B组)和既往术后无内眦赘皮的外折重睑组(C组)。术中通过对重睑线内侧低位画线设计、向内上方的控制缝合以及重睑上方的脂肪转移共3个基本策略,将外折重睑矫正为内折重睑。对比手术前后影像,并评估患者的满意度和并发症。结果102例患者平均随访11.9个月,96例对术后效果满意,外折重睑有效矫正为内折重睑;6例不满意,包括:重睑宽度偏窄、重睑轻度不对称、未完全矫正为内折重睑和重睑折痕内侧细纹。结论采用低位设计、方向控制缝合、脂肪转移等手术策略,可有效地将外折重睑矫正为内折重睑,获得满意的重睑美学效果。Objective To explore a strategy to correct out-fold crease of double eyelid to in-fold to satisfy patients'requirements.Methods From January 2022 to January 2024,102 patients were included in this retrospective analysis.The patients were divided into three groups:inborn out-fold double-eyelid,obtained double-eyelid with epicanthus and obtained double-eyelid without epicanthus.During the operation,the lateral fold double eyelid was corrected to the inner fold double eyelid by three basic strategies:the design of drawing the line inside and low,the control suture inside and above the double eyelid,and the fat transfer above the double eyelid.Preoperative and postoperative images were collected,and patients'satisfaction and complications were evaluated.Results In total,102 patients were included in this study with an average follow-up of 11.9 months.Ninety-six patients were satisfied with their results,their out-fold double eyelid were corrected into in-fold effectively.Six patients were dissatisfied,with dissatisfaction factors including:narrow double eyelid width,asymmetric creases,incompletely in-fold creases,and subtle fork in medial creases.Conclusion The surgical strategy which contained low position design,direction control suture and fat transfer could correct out-fold creases of double-eyelid into in-fold effectively,achieving a more satisfactory aesthetic effect of the double eyelid.
