摘 要:炎症性肠病(inflammatory bowel disease,IBD)主要包括克罗恩病(Crohn disease,CD)和溃疡性结肠炎(ulcerative colitis,UC)两种类型,是一种病因尚不明确的自身免疫疾病,主要特征是慢性肠道炎症。肠道生理屏障完整性受损、化学屏障功能障碍及免疫屏障稳态失衡是慢性肠道炎症发生的病理学基础。在先天性固有免疫中,补体系统扮演着关键的角色,它可以通过保护肠道的生物屏障的完整、化学屏障的功能和免疫屏障的平衡,从而保证肠道的健康状态。目前已有较多研究探讨了血清生物标志物在溃疡性结肠炎活动度评估及优化用药中的临床价值。本文围绕补体系统在IBD发病中的影响及作用机制,对补体因子预测IBD活动度的潜在作用进行阐述。Inflammatory bowel disease(IBD),which mainly includes Crohn disease(CD)and ulcerative colitis(UC),is an autoimmune disease whose cause is not yet known.The main feature is chronic intestinal inflammation.Impairment of intestinal physiological barrier integrity,chemical barrier dysfunction and imbalanced immune barrier homeostasis are the pathological basis of chronic intestinal inflammation.In innate immunity,the complement system plays a key role in maintaining the health of the gut by protecting the integrity of the biological barrier,the function of the chemical barrier and the balance of the immune barrier.At present,many studies have explored the clinical value of serum biomarkers in evaluating activity and optimizing drug use in ulcerative colitis.In this paper,the potential role of complement factors in predicting IBD activity was discussed,focusing on the influence and mechanism of the complement system in the pathogenesis of IBD.
