摘  要:目的:分析太极推拿联合肌电生物反馈疗法治疗脑卒中后上肢运动功能障碍患者的效果。方法:选取2021年6月-2024年3月九江市第一人民医院康复科收治的121例脑卒中后上肢运动功能障碍患者作为研究对象,以随机数字表法分为试验组(n=61)和对照组(n=60)。对照组采取常规康复治疗同时给予肌电生物反馈疗法,试验组在对照组基础上实施太极推拿治疗。比较两组的临床疗效、上肢运动功能、上肢及手功能、患肢肩外展肌肌群、腕背伸肌肌群的肌电值及日常生活活动能力。结果:研究期间对照组有1例患者脱落,最终完成治疗者共120例。治疗后,两组Fugl-Meyer运动功能评估(FMA)评分、上肢功能测试(UEFT)评分、改良Barthel指数(MBI)评分均高于治疗前,且试验组均高于对照组,差异均有统计学意义(P<0.05)。治疗后,两组肩外展肌肌群、腕背伸肌肌群的肌电值均高于治疗前,且试验组均高于对照组,差异均有统计学意义(P<0.05)。结论:太极推拿联合肌电生物反馈疗法治疗,能够有效促进脑卒中后上肢运动功能障碍患者上肢功能、活动度的恢复,改善日常生活活动能力。Objective:To analyze the effect of Taiji massage combined with electromyographic biofeedback therapy in the treatment of upper limb motor dysfunction after stroke.Method:A total of 121 patients with upper limb motor dysfunction after stroke admitted to the Rehabilitation Department of Jiujiang NO.1 People's Hospital from June 2021 to March 2024 were selected as the study objects,and they were divided into experimental group(n=61)and control group(n=60)by random number table method.The control group received routine rehabilitation therapy and electromyographic biofeedback therapy,and the experimental group received Taiji massage therapy on this basis.The clinical efficacy,upper extremity motor function,upper extremity and hand function,the electromyography value of shoulder abductor muscle and dorsal extensor muscle of wrist and the ability of daily living were compared between the two groups.Result:During the study period,1 patient in the control group fell off,and a total of 120 patients completed the treatment.After treatment,Fugl-Meyer assessment of motor function(FMA)score,upper extremity function test(UEFT)score and modified Barthel index(MBI)score in two groups were higher than those before treatment,and those in the experimental group were higher than those in the control group,the differences were statistically significant(P<0.05).After treatment,the myoelectric value of shoulder abductor muscle group and wrist dorsal extensor muscle group in two groups were higher than those before treatment,and those in experimental group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Taiji massage combined with electromyographic biofeedback therapy for patients with upper limb motor dysfunction after stroke can effectively promote the recovery of upper limb function and range of motion,and achieve the purpose of improving the ability of daily living.
