摘　　要：目的探讨不同类型和数量的甲状腺结节钙化与甲状腺癌的关系。方法选取82例甲状腺结节伴钙化患者,将32例被确诊为甲状腺癌的患者作为观察组,另外50例被确诊为甲状腺良性病变的患者作为对照组。比较两组患者甲状腺结节的钙化类型(微小钙化、粗大钙化以及周边钙化)和钙化数量(单发结节钙化、多发结节钙化)。分析甲状腺癌发生的影响因素。结果观察组患者结节微小钙化、周边钙化比例均高于对照组,差异均有统计学意义(P﹤0.05)。观察组患者单发结节钙化比例为75.00%(24/32),多发结节钙化比例为25.00%(8/32),对照组患者单发结节钙化比例为24.00%(12/50),多发结节钙化比例为76.00%(38/50),两组比较,差异有统计学意义(P﹤0.01)。Logistic回归分析结果显示,微小钙化、周边钙化、单发结节钙化均是甲状腺癌发生的危险因素(P﹤0.05)。结论甲状腺结节钙化特征与甲状腺癌存在一定相关性,特别是当出现微小钙化、周边钙化与单发结节钙化时,发生甲状腺癌的危险较大。Objective To investigate the relationship between different types and number of thyroid nodules calcification and thyroid cancer.Method A total of 82 patients with thyroid nodules and calcification were selected.Among them,32 patients with thyroid cancer were selected as the observation group and 50 patients with benign thyroid lesions were selected as the control group.The types of calcification(microcalcification,coarse calcification and peripheral calcification)and the number of calcification(single nodule calcification and multiple nodules calcification)of thyroid nodules were compared between the two groups.The influencing factors of thyroid cancer were analyzed.Result The proportion of microcalcification and peripheral calcification in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).In the observation group,the proportion of single nodule calcification was 75.00%(24/32)and the proportion of multiple nodules calcification was 25.00%(8/32),while in the control group,the proportion of single nodule calcification was 24.00%(12/50)and the proportion of multiple nodules calcification was 76.00%(38/50),and the difference was statistically significant(P<0.01).Logistic regression analysis showed that microcalcification,peripheral calcification,single nodule calcification were risk factors for thyroid cancer(P<0.05).Conclusion There is a certain correlation between the calcification characteristics of thyroid nodules and thyroid cancer,especially when there is microcalcification,peripheral calcification and single nodule calcification,the risk of thyroid cancer is greater.
