摘　　要：目的:观察玻璃体切割术(PPV)联合内界膜翻转填塞术治疗高度近视黄斑裂孔性视网膜脱离(MHRD)患者的效果。方法:回顾性分析2021年7月至2023年6月该院收治的70例高度近视MHRD患者的临床资料,根据手术方式不同将其分成对照组与研究组各35例。对照组采用PPV联合内界膜剥离术治疗,研究组采用PPV联合内界膜翻转填充术治疗,比较两组黄斑裂孔闭合率、视网膜复位率、不同时间点最佳矫正视力(BCVA)水平和并发症发生率。结果:研究组黄斑裂孔闭合率和视网膜复位率均高于对照组,差异有统计学意义(P<0.05);术后1、3、6个月,两组BCVA水平均低于术前,且研究组低于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:PPV联合内界膜翻转填塞术治疗高度近视MHRD患者可提高黄斑裂孔闭合率和视网膜复位率,以及降低BCVA水平的效果优于PPV联合内界膜剥离术治疗。Objective:To observe effects of pars plana vitrectomy(PPV)combined with inverted internal limiting membrane insertion in treatment of high myopia patients with macular hole retinal detachment(MHRD).Methods:The clinical data of 70 high myopia patients with MHRD admitted to this hospital from July 2021 to June 2023 were retrospectively analyzed.According to the different surgical methods,they were divided into control group and study group,35 cases in each group.The control group was treated with PPV combined with internal limiting membrane peeling,while the study group was treated with PPV combined with inverted internal limiting membrane insertion.The macular hole closure rate,the retinal reattachment rate,the best corrected visual acuity(BCVA)levels at different time points,and incidence of complications were compared between the two groups.Results:The macular hole closure rate and the retinal reattachment rate in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).1,3 and 6 months after the surgery,the BCVA levels of the two groups were lower than those before the surgery,those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:PPV combined with inverted internal limiting membrane insertion in the treatment of the high myopia patients with MHRD can improve the macular hole closure rate and the retinal reattachment rate,and reduce the BCVA levels.Moreover,it is superior to PPV combined with internal limiting membrane peeling.
