十二指肠癌及结直肠癌组织中CDX2的表达与病理特征和预后的关系

摘　　要：目的分析十二指肠癌(DAD)及结直肠癌(CRC)组织中尾型同源盒基因2(CDX2)的表达与病理特征和预后的关系。方法收集本院2017年1月至2021年6月经本院诊治的52例DAD和CRC患者,所有患者均行DAD和CRC根治术。通过免疫组化检测CDX2在DAD和CRC中的表达,应用Log-rank检验行生存分析,应用Cox风险比例回归模型行多因素分析。结果本研究中,CDX2的表达与TNM分期、分化程度有关,TNM分期越晚CDX2阳性表达率越低,分化程度越高CDX2阳性表达率越高(P<0.05),性别、年龄以及神经侵犯情况与CDX2的表达无关(P>0.05)。经单因素分析,十二指肠癌、结直肠癌患者的的预后与TNM分期、分化程度及CDX2表达相关(P<0.05),经Cox多因素分析,分化程度和CDX2的表达是影响十二指肠癌、结直肠癌的独立预后因素(P<0.05)。结论TNM分期、CDX2的表达和分化程度与CRC和DAD患者的术后预后呈显著相关关系,Cox多因素分析可得CDX2的表达和分化程度是CRC和DAD患者的预后的独立因素,为临床诊治开展新思路。Objective To analyze the relationship between CDX2 expression in duodenal adenocarcinoma(DAD)and colorectal cancer(CRC)tissues with pathological features and prognosis.Methods 52 patients with DAD and CRC who were diagnosed and treated in our hospital from January 2017 to June 2021 were collected,and all of them underwent radical surgery for DAD or CRC.The expression of CDX2 in DAD and CRC was detected by immunohistochemistry,Log-rank test was applied to perform survival analysis,and Cox risk-proportional regression model was applied to perform multifactorial analysis.Results In this study,the expression of CDX2 was related to the TNM stage and differentiation degree,the later the TNM stage,the lower the positive expression rate of CDX2,and the higher the differentiation degree,the higher the positive expression rate of CDX2(P<0.05),and gender,age,and neural invasion were not related to the expression of CDX2(P>0.05).By univariate analysis,the prognosis of patients with duodenal cancer and colorectal cancer was associated with TNM stage,degree of differentiation and CDX2 expression(P<0.05),and by Cox multivariate analysis,the degree of differentiation and CDX2 expression were independent prognostic factors affecting duodenal cancer and colorectal cancer(P<0.05).Conclusion TNM stage,expression of CDX2 and degree of differentiation were significantly correlated with the postoperative prognosis of patients with CRC or DAD,and Cox multifactorial analysis can be obtained that expression of CDX2 and degree of differentiation are independent prognostic factors for patients with CRC or DAD,which carries out a new way of thinking for clinical diagnosis and treatment.
