摘　　要：目的探索帕金森病患者的认知功能下降是否与大脑皮层结构的偏侧化改变有关。方法从Parkinson′s Progression Markers Initiative(PPMI)数据库中纳入81例帕金森病伴轻度认知障碍(PDMCI)患者和134例认知功能正常的帕金森病(PDNC)患者,对比两组间各脑区不同形态学参数的偏侧化指数(LI)差异,进一步探讨神经心理学测试与差异脑区形态学参数LI之间的相关关系,最后利用Logistic回归模型评估其对PDMCI的诊断效能。结果PDMCI患者与PDNC患者间存在岛短回、额下回、胼胝体下回、横额极回、额中沟和顶下沟等多个脑区皮层结构的偏侧化差异,部分脑区的LI与神经心理学量表评分存在相关性,并对PDMCI的诊断具有一定价值。结论帕金森病不同认知功能的患者存在大脑皮质结构的偏侧化差异,不同脑区的皮层结构偏侧化可能提示了不同认知领域的改变,这为后续的研究提供了新的视角。Objective To explore whether the decline in cognitive function in patients with Parkinson's disease is associated with the lateralization of cerebral cortex structure.Methods From the Parkinson's Progression Markers Initiative(PPMI)database,81 patients with Parkinson's disease with mild cognitive impairment(PDMCI)and 134 patients with Parkinson's disease(PDNC)with normal cognitive function were included.The differences in lateralization index(LI)of different morphological parameters in various brain regions between the two groups were compared.The correlation between neuropsychological tests and the LI of morphological parameters in these brain regions was further explored.Logistic regression was used to evaluate the diagnostic efficacy for PDMCI.Results Lateralization differences were found between PDMCI and PDNC patients in several brain regions,including the short insula gyrus,inferior frontal gyrus,subcallosal gyrus,transverse frontal gyrus,middle frontal sulcus,and subparietal sulcus.The lateralization index(LI)of some brain regions correlated with the scores of neuropsychological scales and showed diagnostic value for PDMCI.Conclusion Patients with Parkinson's disease and different cognitive functions exhibit distinct lateralization of cerebral cortex structure.The structural lateralization in different brain regions may indicate changes in various cognitive domains,offering a new perspective for future research.
