摘 要:目的探索屈光参差性弱视儿童大脑皮层形态学的变化。方法33例屈光参差性弱视儿童和24名年龄、性别匹配的健康对照儿童进行3.0T MRI扫描。应用基于表面的形态学测量(SBM)方法进行数据处理,采用双样本t检验分别比较两组间皮层厚度(CT)、沟深(SD)、脑回指数(GI)和分形维数(FD)的差异。结果与健康对照组相比,屈光参差性弱视组在右侧颞中回、左侧颞下回、左侧缘上回、双侧额上回和双侧枕外侧的CT减少,在左侧颞中回、左侧颞下回、左侧楔叶和左侧海马旁回的SD减少,在左侧海马旁回和右侧中央前回的GI增加,在右侧外侧眶额皮质和双侧枕外侧的FD减少(均P<0.05)。结论除了枕叶视皮层,屈光参差性弱视儿童在额、顶、颞叶的皮层厚度和折叠模式同样存在异常,为弱视的中枢病理改变提供了解剖学证据。Objective To explore the cortical morphological changes in children with anisometropic amblyopia.Methods Thirty-three children with anisometropic amblyopia and 24 age-and sex-matched healthy controls underwent 3.0T MRI scanning.Surface-based morphometry(SBM)was used for data processing,and two-sample t-tests were used to compare the differences in cortical thickness(CT),sulcal depth(SD),gyrification index(GI),and fractal dimension(FD)between the two groups.Results Compared with the healthy control group,the anisometropic amblyopia group showed decreased CT in the right middle temporal gyrus,left inferior temporal gyrus,left supramarginal gyrus,bilateral superior frontal gyrus,and bilateral lateral occipital gyrus;decreased SD in the left middle temporal gyrus,left inferior temporal gyrus,left cuneus,and left parahippocampal gyrus;increased GI in the left parahippocampal gyrus and right precentral gyrus;and decreased FD in the right lateral orbitofrontal cortex and bilateral lateral occipital cortex(all P<0.05).Conclusion In addition to the occipital visual cortex,there are also abnormalities in the cortical thickness and folding patterns of the frontal,parietal,and temporal lobes in children with anisometropic amblyopia,providing anatomical evidence for central pathological changes in amblyopia.
