广东青少年学生吸卷烟行为现状及影响因素分析

摘　　要：目的 了解广东青少年学生吸卷烟行为情况,探索其影响因素,为推动青少年控烟工作提供科学依据。方法 采用多阶段分层整群随机抽样法,抽取广东省10个县(区)58所中学(初中、普通高中和职业高中)学生进行问卷调查,运用统计软件SPSS 23.0复杂抽样分析程序,采用单因素和多因素分析探讨影响因素。结果 共调查青少年学生8 860人,其中男生4 689人,女生4 171人,年龄以13~18岁为主(89.9%)。广东省青少年学生现在吸卷烟率4.9%,男生(7.4%)高于女生(2.0%),初中、普通高中、职业高中学生分别为2.3%、4.1%、17.7%;青少年尝试吸卷烟率17.8%,男生(24.8%)高于女生(9.7%),初中、普通高中、职业高中分别为11.3%、21.8%、37.4%(均P<0.05)。有68.2%的学生在家、室内、室外公共场所或公共交通工具4类公共场所处于二手烟暴露,有46.9%的学生在学校暴露于二手烟。男生(OR=2.434、2.137)、高中学段(OR=1.873、1.793)、好朋友吸烟(OR=31.681、4.566)、老师吸烟(OR=1.777、1.614)和家庭二手烟暴露(OR=1.706、1.410)均是青少年学生现在吸卷烟和尝试吸卷烟的危险因素(均P<0.05),30 d内接触过控烟信息(OR=0.616、0.758)则是现在吸卷烟和尝试吸卷烟的阻碍因素(均P<0.05)。结论 广东青少年尝试吸卷烟率较高,男生、高中生是吸卷烟重点人群。吸烟行为受性别、学段、重要他人吸烟和二手烟暴露影响,应采取综合干预措施减少学生烟民产生。Objective To explore the status of cigarette smoking and its influencing factors among adolescent stu‑dents in Guangdong Province,so as to provide a scientific basis for promoting tobacco control among adolescents.Methods Using a multi-stage stratified cluster random sampling method,selected students from 58 middle schools(in‑cluding junior high schools,ordinary high schools,and vocational high schools)in 10 counties or districts of Guangdong Province for self-administered questionnaires.Univariate and multivariate analyses with complex survey module of SPSS 23.0 were used to explore the influencing factors.Results A total of 8860 students were investigated,including 4689 boys and 4171 girls,mainly aged 13 to 18 years,accounting for 89.9%.The rate of cigarette smoking among adolescent stu‑dents was 4.9%in Guangdong Province,with boys(7.4%)significantly higher than girls(2.0%),and the rates of junior high school,ordinary high school,and vocational high school students were 2.3%,4.1%,and 17.7%,respectively(P<0.05).The rate of cigarette smoking attempt among adolescent students was 17.8%,with boys(24.8%)significantly higher than girls(9.7%),and the rates of junior high school,ordinary high school,and vocational high school students were 11.3%,21.8%,and 37.4%,respectively(P<0.05).There were 68.2%of students were exposed to secondhand smoke in four types of public places:at home,indoors,outdoors,or public transport.In addition,46.9%of students were exposed to secondhand smoke at school.Logistic regression analysis showed that boys(OR=2.434,2.137),high school period(OR=1.873,1.793),peer smok‑ing(OR=31.681,4.566),teachers smoking(OR=1.777,1.614),and secondhand smoke exposure in the home(OR=1.706,1.410)were the common risk factors of cigarette smoking and smoking attempt behaviors among adolescent students.Expo‑sure to tobacco control information within 30 days(OR=0.616,0.758)was the factor that impede cigarette smoking and smoking attempt behaviors.Conclusions The rate of cigarette smoking attempt is higher am
