阴道镜检查联合阴道镜Reid评分在子宫颈癌前病变诊断中的临床应用

摘　　要：目的 探讨阴道镜检查联合阴道镜Reid评分在子宫颈癌前病变诊断中的临床价值。方法选取2022年7月—2023年10月兰州市妇幼保健院就诊的300例疑似子宫颈癌前病变患者作为研究对象,均开展阴道镜检查并进行阴道镜Reid评分,以病理检查结果为金标准,分析阴道镜检查与阴道镜检查联合阴道镜Reid评分诊断子宫颈癌前病变的效能和诊断符合率。结果 阴道镜检查联合阴道镜Reid评分诊断子宫颈癌前病变的灵敏度、准确度、阴性预测值均高于阴道镜检查,差异有统计学意义(P均<0.05)。300例受检者经病理检查检出宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)125例,其中阴道镜检查对CIN的诊断总符合率为92.00%(115/125),低于阴道镜检查联合阴道镜Reid评分的98.40%(123/125),差异有统计学意义(χ^(2)=5.602,P=0.018)。结论 阴道镜检查联合阴道镜Reid评分可提高子宫颈癌前病变诊断价值,便于早期开展针对性治疗,降低宫颈癌发生风险。Objective To explore the clinical value of colposcopy combined with colposcopy Reid score in the diagnosis of cervical precancerous lesions.Methods A total of 300 patients with suspected cervical precancerous lesions who were treated in Lanzhou Maternal and Child Health Hospital from July 2022 to October 2023were selected as the research objects.Colposcopy was performed and colposcopy Reid score was performed.Pathological examination result was used as the gold standard.The efficacy and the diagnostic coincidence rate of colposcopy and colposcopy combined with colposcopy Reid score in the diagnosis of cervical precancerous lesions was analyzed.Results The sensitivity,accuracy and negative predictive value of colposcopy combined with colposcopy Reid score in the diagnosis of cervical precancerous lesions were higher than those of colposcopy,and the differences were statistically significant(all P<0.05).Among the 300 subjects,125 cases of cervical intraepithelial neoplasia(CIN) were detected by pathological examination,and the total diagnostic coincidence rate of colposcopy for CIN was 92.00%(115/125),which was lower than that of colposcopy combined with colposcopy Reid score 98.40%(123/125),the difference was statistically significant(χ~2=5.602,P=0.018).Conclusion Colposcopy combined with colposcopy Reid score can improve the diagnostic value of cervical precancerous lesions,facilitate early targeted treatment,and reduce the risk of cervical cancer.
