9例圆房失败伴儿童期性侵害女性患者的临床报道

摘　　要：目的通过对9例圆房失败伴儿童期性侵害女性患者的临床资料进行分析,探讨该疾病的临床特征及诊治方法。方法选取2020年1月至2023年7月在南京某三甲医院就诊的9例圆房失败伴儿童期性侵害女性患者作为研究对象,对其进行一般资料调查表、自制的儿童期性侵害经历调查表、女性性功能量表(FSFI)、性厌恶量表、性交恐惧诊断、阴道痉挛分级调查。性治疗师采用Plisst模型、认知行为疗法改变患者性观念,同时采用放松训练、系统脱敏疗法和性感集中训练,帮助患者克服性交恐惧和性厌恶,实现圆房。结果患者平均年龄(28.56±3.16)岁,平均病程(2.31±1.52)年。FSFI显示4例性欲低下,6例性唤起障碍,7例阴道润滑障碍,8例性交痛。性厌恶量表提示4例性厌恶,9例均确诊为性交恐惧,其中8例确诊为阴道痉挛。经过治疗,8例圆房成功,1例圆房失败后离婚。结论圆房失败伴儿童期性侵害女性患者在成年后受到性刺激会导致不良感受重现,出现女性性功能障碍,导致夫妻圆房失败。性治疗师通过Plisst模型、认知行为疗法、放松训练、系统脱敏疗法和性感集中训练等可以帮助患者树立正确的性观念,克服性交恐惧,改善夫妻亲密关系,帮助圆房成功。Objective To analyze the clinical data of 9 female patients with unconsummated marriage and childhood sexual assault,and to discuss the clinical characteristics,diagnosis and treatment methods of the disease.Methods A total of 9 female patients with unconsummated marriage and childhood sexual assault who were admitted a tertiary grade A hospital in Nanjing from January 2020 to July 2023 were selected as research subjects.The general data questionnaire and the homemade childhood sexual assault experience questionnaire were conducted.Female sexual function index(FSFI),sexual aversion scale,diagnosis of vaginal penetration phobia,and grading survey of vaginismus were conducted.Sex therapists use Plisst model and cognitive behavioral therapy to change patients′sexual concepts,and at the same time relaxation training,systematic desensitization therapy and sensate focus training were adapted to help patients overcome vaginal penetration phobia and sexual aversion,and complete intercourse.Results The average age of the patients were(28.56±3.16)years and the mean course of the disease were(2.31±1.52)years.FSFI showed low sexual desire in 4 cases,sexual arousal disorder in 6 cases,vaginal lubrication disorder in 7 cases,and sexual intercourse pain in 8 cases.The sexual aversion scale indicated that 4 cases of sexual aversion,9 cases were diagnosed with vaginal penetration phobia,and 8 of them were diagnosed with vaginismus.After treatment,8 cases successfully completed the intercourse,and 1 case divorced after failed.Conclusions Female patients with unconsummated marriage and childhood sexual assault undergoing sexual stimulation in adulthood will lead to reappearance of adverse feelings,female sexual dysfunction,and lead to unconsummated marriage.Through Plisst models,cognitive behavioral therapy,relaxation training,systematic desensitization therapy and sensate focus training,etc.,sex therapists can help patients establish correct sexual concepts,overcome vaginal penetration phobia,improve the intimacy between husba
