摘　　要：目的:分析广西三级公立医院运营效率,查找“国考”背景下医院运营管理中的问题。方法:基于“国考”运营效率监测指标构建综合评价指标体系,采用DEA-BCC、Malmquist指数模型对指标进行分析,运用Tobit回归模型分析影响公立医院运营效率的主要因素。结果:2020—2022年,广西60家三级公立医院纯技术效率总体保持较高水平;Malmquist指数处于下降状态,技术进步指标是医院运营效率下降的主要影响因素;Tobit回归显示,床位规模、门诊与住院患者人次比、出院患者手术占比、业务支出和年总收入对医院运营效率具有显著影响。结论:广西三级公立医院规模不合理问题突出,资源利用效率有待提升,运营管理情况不容乐观;建议发挥“国考”的导向作用,加快推动医院发展方式向质量效益型转变,调整卫生资源战略布局,积极落实分级诊疗,在控制业务成本费用的同时合理提高人员支出占比,从而提高医院整体运营水平。Objective To analyze the operational efficiency of the tertiary public hospitals in Guangxi,and to find out the problems in the operational management of the hospitals under the background of "national examination".Methods Based on the monitoring indexes of operational efficiency of "national examination",a comprehensive evaluation index system was constructed.The DEA-BCC and Malmquist index models were used to analyze the static and dynamic indexes.The Tobit regression model was used to analyze the main factors affecting the operational efficiency of public hospitals.Results From 2020 to 2022,the pure technical efficiency of 60 tertiary public hospitals in Guangxi maintained a relatively high level in general.The Malmquist index was in a state of decline,and the technical progress index was the main influencing factor for the decline of hospital operational efficiency.The Tobit regression showed that the size of the beds,the ratio of outpatient and inpatient attendances,the percentage of surgery for discharged patients,the business expenditures and annual total income had a significant impact on the operational efficiency of hospitals.Conclusion The tertiary public hospitals in Guangxi have outstanding problems of unreasonable scale,the efficiency of health resource utilization needs to be improved,and the operational management situation is not optimistic.It is necessary to play the guiding role of "national examination",accelerate the transformation of the development mode of hospitals to the quality and efficiency mode,adjust the strategic layout of health resources,actively implement the hierarchical diagnosis and treatment,and reasonably increase the proportion of personnel expenditure while controlling the business costs,so as to improve the overall operational level of the hospital.
