摘 要:目的:分析微小核糖核酸-223(miR-223)、血小板分布宽度(PDW)联合红细胞分布宽度(RDW)预测乙型肝炎病毒(HBV)相关性肝癌患者导管肝动脉化疗栓塞(TACE)术后抗病毒治疗反应性效能的关系。方法:选择2019年1月至2022年1月收治的HBV相关性肝癌患者128例,所有患者均接受TACE联合核苷(酸)类似物抗病毒治疗。治疗后随访12个月,记录HBV激活情况。患者入院时均接受miR-223、PDW、RDW检测并记录基线资料。采用Pearson相关性分析miR-223、PDW、RDW之间的关系。采用二元Logistics回归分析miR-223相对表达量、PDW、RDW与HBV相关性肝癌患者抗病毒疗效的关系。采用接受者操作特征曲线(ROC)分析miR-223相对表达量、PDW、RDW水平预测HBV相关性肝癌患者抗病毒疗效的价值。结果:抗病毒治疗12个月后,19例(14.84%)HBV激活患者为激活组,109例(85.16%)HBV未激活患者为未激活组,两组患者肿瘤数目、Child-Pugh分级、基线HBV DNA水平比较差异无统计学意义(P>0.05);采用Pearson相关性分析发现,miR-223相对表达量与PDW、RDW无明显相关性(P>0.05),PDW与RDW无明显相关性(P>0.05);行二元Logistics回归分析结果显示,miR-223相对表达量、PDW、RDW是HBV相关性肝癌患者抗病毒治疗后HBV激活的危险因素(OR=1.166、1.831、1.430,P<0.05);ROC曲线显示,入院时miR-223相对表达量、PDW、RDW水平及联合检测预测HBV相关性肝癌患者抗病毒治疗无效的AUC分别为0.878、0.714、0.763、0.955,其中联合检测最高。结论:miR-223相对表达量、PDW及RDW水平与HBV相关性肝癌患者抗病毒治疗反应性效能关系密切,三者及联合检测能够有效预测HBV相关性肝癌患者抗病毒治疗效果。Objective:The relationship between microRNA-223(miR-223),platelet distribution width(PDW),and red blood cell distribution width(RDW)in predicting the reactivity efficacy of antiviral therapy after transcatheter arterial chemoembolization(TACE)in patients with hepatitis B virus(HBV)-associated liver cancer.Methods:128 patients with HBV-related liver cancer admitted to our hospital from January 2019 to January 2022 were selected.All patients received transcatheter hepatic arterial chemoembolization(TACE)combined with antiviral therapy with nucleoside(acid)analogues.The patients were followed up for 12 months after treatment and HBV activation was recorded.MiR-223,PDW and RDW were measured and baseline data were recorded at the time of admission.The relation between miR-223,PDW and RDW was analyzed by Pearson correlation.Binary logistics regression analysis showed the relationship between the relative expression of miR-223,PDW and RDW and the antiviral efficacy of HBV-related liver cancer patients.The receiver operating characteristic curve(ROC)was used to analyze the relative expression of miR-223,and the value of PDW and RDW levels in predicting the antiviral efficacy in patients with HBV-related liver cancer.Results:After 12 months of antiviral treatment,19 cases(14.84%)had active HBV and 109 cases(85.16%)had not.The relative expression levels of miR-223,PDW and RDW in activated group were higher than those in non-activated group(P<0.05).There were no significant differences in age,sex,BMI,tumor number,Child-Pugh grade and baseline HBV DNA level between the two groups(P>0.05).Using Pearson correlation test,there was no obvious correlation between miR-223 and PDW and RDW(P>0.05),and no obvious correlation between PDW and RDW(P>0.05).The results of binary logistics regression analysis show that,the relative expression levels of miR-223,PDW and RDW were the risk factors for HBV activation in HBV-associated liver cancer patients after antiviral therapy(OR=1.166,1.831,1.430,all P<0.05).The results of the ROC curve sho
