血清CEA、CA-199及RBP4表达与肝内胆管结石患者术后复发的相关性

摘　　要：目的:探讨血清癌胚抗原(CEA)、糖类抗原199(CA-199)及视黄醇结合蛋白4(RBP4)表达与肝内胆管结石(IHDS)患者术后复发的相关性。方法:选取2021年2月至2023年2月收治的80例IHDS手术治疗的患者作为研究对象,根据术后6个月是否复发IHDS,将其分为复发组和非复发组。比较两组患者基础资料,血清CEA、CA-199及RBP4水平表达与IHDS术后复发的相关性。结果:80例IHDS手术患者,术后有12例复发,复发率为15.0%(12/80);Logistic回归分析结果显示,血清CEA、CA-199以及RBP4为IHDS术后复发的危险因素;相关性分析结果显示,CEA、CA-199以及RBP4与IHDS术后复发均呈正相关;血清RBP4水平预测IHDS术后复发的曲线下面积(AUC)值为0.801,CEA水平预测IHDS术后复发的AUC值为0.746,CA-199水平预测IHDS术后复发的AUC值为0.744,联合预测IHDS术后复发的AUC值为0.886。结论:IHDS患者术后具有较高的复发率,且术后复发与血清CEA、CA-199以及RBP4水平过表达密切相关。临床中可通过检测IHDS术后患者血清CEA、CA-199以及RBP4水平,预测其复发风险,为临床提供更准确的诊断和治疗方案。Objective:To investigate the correlation between serum CEA,CA-199 and RBP4 expression and postoperative recurrence of intrahepatic bile duct stones.Methods:A total of 80 patients with intrahepatic cholangiolithiasis(IHDS)treated by surgery in our hospital from February 2021 to February 2023 were selected as the study objects.Patients were divided into recurrence and non-recurrence groups based on whether IHDS recurred after 6 months post-surgery.The baseline data,serum levels of CEA,CA-199,and RBP4 expression were compared between the two groups to analyze their correlation with postoperative recurrence of IHDS.Results:Among 80 IHDS surgical patients,12 experienced recurrence postoperatively,with a recurrence rate of 15.0%(12/80).The results of logistic regression analysis showed that serum CEA,CA-199,and RBP4 are risk factors for postoperative recurrence of IHDS.Correlation analysis showed that CEA,CA-199 and RBP4 were positively correlated with postoperative recurrence of IHDS.Serum RBP4 level predicted postoperative recurrence of IHDS with an AUC value of 0.801,CEA level predicted postoperative recurrence of IHDS with an AUC value of 0.746,CA-199 level predicted postoperative recurrence of IHDS with an AUC value of 0.744,and combined predicted postoperative recurrence of IHDS with an AUC value of 0.886.Conclusion:Patients with IHDS have a high recurrence rate after surgery,and postoperative recurrence is closely related to the overexpression of serum CEA,CA-199 and RBP4 levels.In clinical practice,the risk of recurrence can be predicted by detecting the serum CEA,CA-199 and RBP4 levels of patients with IHDS after surgery,so as to provide more accurate clinical diagnosis and treatment.
