摘 要:目的探讨血清沉默信息调节因子相关酶1(SIRT1)、过氧化物酶体增殖物激活受体γ辅助激活因子-1α(PGC-1α)与突发性聋患者血清炎症因子水平以及听力预后的相关性。方法选取2020年5月—2022年5月牡丹江市第二人民医院收治的88例突发性聋患者作为研究组,根据患者听力恢复情况分为预后良好组和预后不良组,另收集牡丹江市第二人民医院进行体检的健康体检者82例为对照组。采用酶联免疫吸附法检测患者血清SIRT1、PGC-1α以及炎症因子-肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、超敏C反应蛋白(hs-CRP)水平。Pearson法分析患者血清SIRT1、PGC-1α水平与炎症因子水平的相关性。Logistic回归分析听力预后的影响因素。结果突发性聋患者有高血压史、糖尿病史比例高于对照组,SIRT1、PGC-1α水平显著低于对照组,hs-CRP、IL-1β、TNF-α水平显著高于对照组(P<0.05);突发性聋患者SIRT1水平与hs-CRP、IL-1β、TNF-α水平分别呈负相关(r=-0.524,-0.384,-0.542,均P<0.05),PGC-1α水平与hs-CRP、IL-1β、TNF-α水平分别呈负相关(r=-0.429,-0.457,-0.624,均P<0.05);其中预后不良组hs-CRP、IL-1β、TNF-α水平显著高于预后良好组,SIRT1、PGC-1α水平显著低于预后良好组,有高血压史、糖尿病史比例高于预后良好组(P<0.05);Logistic回归分析显示,SIRT1、PGC-1α、hs-CRP、IL-1β、TNF-α、高血压史、糖尿病史是突发性聋患者听力预后的影响因素(P<0.05)。结论SIRT1、PGC-1α在突发性聋患者血清中呈低表达,二者与血清炎性因子密切相关,且是听力预后的影响因素。Objective To investigate the correlation of serum silence information regulator homolog 1(SIRT1),peroxisome proliferator-activated receptorγcoactivator-1α(PGC-1α)with serum inflammatory factors and hearing prognosis in patients with sudden deafness.Methods A total of 88 patients with sudden deafness admitted to the Second People’s Hospital of Mudanjiang City from May 2020 to May 2022 were taken as the study group,and divided into the good prognosis group and the poor prognosis group according to their hearing recovery.In addition,82 healthy people who underwent physical examination in the hospital were collected as the control group.Serum levels of SIRT1,PGC-1α,inflammatory factor-tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and hypersensitive C-reactive protein(hs-CRP)were detected by enzyme-linked immunosorbent assay.Pearson method was adopted to analyze the correlation between serum levels of SIRT1,PGC-1αand inflammatory factors.Logistic regression analysis was applied to analyze the influencing factors of hearing prognosis.Results The proportions of patients with history of hypertension and diabetes in the study group were higher than those in the control group,the levels of SIRT1 and PGC-1αof the study group were significantly lower than those of the control group,and the levels of hs-CRP,IL-1βand TNF-αof the study group were obviously higher than those of the control group(all P<0.05).The level of SIRT1 in patients with sudden deafness was negatively correlated with the levels of hs-CRP,IL-1βand TNF-α(r=-0.524,-0.384,-0.542,all P<0.05),and the level of PGC-1αwas negatively correlated with the levels of hs-CRP,IL-1βand TNF-α(r=-0.429,-0.457,-0.624,all P<0.05).In patients with sudden deafness,the levels of hs-CRP,IL-1βand TNF-αof the poor prognosis group were significantly higher than those of the good prognosis group,the levels of SIRT1 and PGC-1αof the poor prognosis group were obviously lower than those of the good prognosis group,and the proportions of patients with hypertensi
