视听反馈式实时重读运动疗法对痉挛型脑性瘫痪儿童言语流利性障碍的疗效研究

摘  要:目的:探讨视听反馈式实时重读运动疗法对痉挛型脑瘫儿童言语流利性障碍的临床治疗效果。方法:选取符合入组标准的44例5—7岁痉挛型脑瘫儿童为研究对象,按照随机数表法将其分为试验组与对照组,每组22例。对照组进行传统重读运动疗法训练,试验组在对照组基础训练内容的基础上进行视听反馈式实时重读运动疗法训练,每组患者共训练10次,1次/日,30min/次,3周内完成。选择符合生态效度的看图说话主题图片《做家务》作为测试材料,收集两组被试前测与后测语音样本,采用双组前后测试验设计,比较两组患者语速、停顿、拖延与重复的言语流利性客观指标的前后差异,评估其临床效果。结果:通过两组治疗前组间比较,言语流利性功能的语速、停顿、拖延与重复的测量指标均无显著性差异(P>0.05),具有可比性;通过两组治疗后组间比较,言语流利性功能7个测量指标均有显著性差异(P<0.05),试验组显著优于对照组;通过两组治疗前后组内比较,试验组治疗前后言语流利性功能7个测量指标均有显著性差异(P<0.05),治疗后显著优于治疗前;而对照组治疗前后均无显著性差异(P>0.05),未见明显变化。结论:通过视听反馈式实时重读运动疗法训练,痉挛型脑瘫儿童语速提升,异常停顿、拖延与重复情况减少,言语流利性功能取得了明显改善,说明视听反馈式实时重读运动疗法对改善痉挛型脑瘫儿童言语流利性障碍具有重要效果,建议未来开展多中心研究与临床推广应用。Objective:To investigate the clinical effect of audio-visual feedback real-time accent movement therapy on speech fluency disorders in children with spastic cerebral palsy.Method:A total of 44 children aged 5—7 years with spastic cerebral palsy who met the inclusion criteria were randomly assigned to an experimental group(n=22)or a control group(n=22)using a random number table method.The control group received traditional accent movement therapy,while the experimental group received audio-visual feedback real-time accent movement therapy in addition to the control group’s training regimen.Both groups completed 10 sessions over 3 weeks(one session per day,30 minutes per session).Speech samples were collected pre-and post-treatment using the picture-based narrative task Doing Housework,selected for its ecological validity.A two-group pretest-posttest design was employed to evaluate changes in objective speech fluency metrics,including speech rate,pauses,prolongations,and repetitions,and to assess the clinical efficacy of the intervention.Result:Before treatment,there were no significant differences between the two groups in seven speech fluency metrics(P>0.05),including speech speed,pause,prolongation,and repetition,indicating comparability.After treatment,there was a significant difference in the 7 measurement indicators of speech fluency function between the two groups(P<0.05),and the experimental group was significantly better than the control group(P<0.05).Within-group comparisons revealed significant improvements in speech fluency metrics in the experimental group after treatment(P<0.05),whereas no significant changes were observed in the control group(P>0.05).Conclusion:Audio-visual feedback real-time accent movement therapy significantly improved speech fluency in children with spastic cerebral palsy,as evidenced by increased speech rate and reduced abnormal pauses,prolongations,and repetitions.These findings indicates that this therapy is an effective intervention for improving speech fluency disorders
