肝内胆管癌患者MK蛋白和PRR11及CPS1的表达及意义

摘  要:目的:分析肝内胆管癌(ICC)患者中期因子(MK)蛋白、富含脯氨酸蛋白11(PRR11)及氨甲酰磷酸合成酶(CPS1)的表达及意义。方法:选取2020年1月至2022年1月郑州市中心医院收治的132例ICC患者,比较胆管癌组织和癌旁组织中MK、PRR11及CPS1的表达情况;分析胆管癌组织中MK、PRR11及CPS1的表达与病理特征的关系;采用KaplanMeier生存曲线分析MK蛋白、PRR11及CPS1表达与胆管癌患者生存时间关系;分析影响ICC患者预后死亡的单因素,采用COX风险回归分析ICC死亡的危险因素。结果:胆管癌组织中MK、PRR11及CPS1阳性表达率高于癌旁组织(P<0.05)。TNM分期为Ⅲ~Ⅳ期、分化程度为中、低分化、有淋巴结转移、肿瘤长径≥3 cm及浸润深度侵及外膜的ICC患者MK、PRR11及CPS1阳性表达高于TNM分期为Ⅰ~Ⅱ期、分化程度为高分化、无淋巴结转移、肿瘤长径<3 cm及浸润深度未及外膜患者(P<0.05)。有事件组(随访期间出现疾病的复发、转移或死亡者)TNM分期Ⅲ~Ⅳ期、分化程度中、低分化、有淋巴结转移及MK、PRR11、CPS1阳性表达、肿瘤长径≥3 cm及侵及外膜占比显著高于无事件生存组(P<0.05)。Logistic分析表明,TNMⅢ~Ⅳ期、中低分化、淋巴结转移、MK、PRR11、CPS1阳性、肿瘤长径≥3 cm及浸润深度侵及外膜为ICC患者预后有事件组的独立危险因素(P<0.05)。结论:PRR11、CPS1和MK蛋白在ICC组织中高表达,且其表达情况与ICC的临床分期、病理分级和淋巴结转移情况紧密相关,上述蛋白的阳性表达是影响ICC患者预后死亡的独立危险因素,能够为预测患者的预后提供一定参考信息。Objective:To analyze the expression and significance of midkine(MK)protein,proline-rich protein 11(PRR11)and carbamyl phosphate synthase(CPS1)in patients with intrahepatic cholangiocarcinoma(ICC).Methods:A total of 132 patients with ICC admitted to Zhengzhou Central Hospital from January 2020 to January 2022 were selected.The relationship between the expression of MK,PRR1l and CPS1 in cholangiocarcinoma tissues and pathological features was analyzed.Kaplan-Meier survival curve was used to analyze the relationship between MK protein,PRR1l and CPSI expression and the survival time of patients with cholangiocarcinoma.The single factors affecting the prognosis and death of ICC patients were analyzed,and the risk factors of ICC death were analyzed by COX risk regression.Results:The positive expression rates of MK,PRR11 and CPS1 in cholangiocarcinoma tissues were higher than those in tumor-adjacent tissues(P<0.05).The positive expression of MK,PRR11 and CPS1 in ICC patients with TNM stageⅢtoⅣ,moderately and poorly differentiated degree,lymph node metastasis,tumor length≥3 cm and infiltration depth invading the outer membrane was higher than that in ICC patients with TNM stageⅠtoⅡ,well differentiated degree,no lymph node metastasis,tumor length<3 cm and no infiltration depth invading the outer membrane(P<0.05).The proportion of patients with TNM stageⅢtoⅣ,moderately or poorly differentiated,lymph node metastasis,positive expressions of MK,PRR1l and CPS1,tumor length≥3 cm and invasion of the outer membrane in the event group(recurrence,metastasis or death of the disease during the follow-up)was significantly higher than that in the event-free survival group(P<0.05).Logistic analysis showed that TNM stageⅢtoⅣ,middle and low differentiation,lymph node metastasis,MK,PRR1l,CPS1 positive,tumor length≥3 cm and invasion depth into the outer membrane were independent risk factors for the death group ofICC patients(P<0.05).Conclusions:The proteins PRR1l,CPS1,and MK are highly expressed in ICC tissues,and the
