血清miR-15a、miR-24水平对功能性垂体腺瘤患者术后复发的预测效能

摘  要:目的探讨血清miR-15a、miR-24水平对功能性垂体腺瘤患者术后复发的预测效能。方法选取2017年6月至2022年6月于西安国际医学中心医院手术治疗的107例功能性垂体腺瘤患者作为研究对象,术后随访2年,根据是否复发将患者分为复发组(n=22)和未复发组(n=85)。收集研究对象临床资料,并在术后5 d采用实时荧光定量PCR方法检测患者血清miR-15a、miR-24水平。采用受试者工作特性(ROC)曲线分析血清miR-15a、miR-24对功能性垂体腺瘤患者术后复发的预测价值;采用多因素Logistic回归分析功能性垂体腺瘤患者术后复发的影响因素。结果复发组患者血清miR-15a、miR-24水平均明显低于未复发组患者(P<0.05)。血清miR-15a、miR-24预测功能性垂体腺瘤患者术后复发的曲线下面积(AUC)分别为0.781(95%CI:0.736~0.831)、0.819(95%CI:0.774~0.869),两者联合预测的AUC为0.906(95%CI:0.856~0.951)。单因素分析结果显示:复发组患者肿瘤分期Ⅲ~Ⅳ期比例、病理类型侵袭比例明显高于未复发组(P<0.05)。多因素回归分析显示:肿瘤分期Ⅲ~Ⅳ期(OR=2.307,95%CI:1.433~3.715)、病理类型侵袭(OR=2.140,95%CI:1.266~3.619)、miR-15a≤0.52(OR=2.795,95%CI:1.820~4.294)、miR-24≤0.91(OR=2.649,95%CI:1.704~4.116)是功能性垂体腺瘤患者术后复发的危险因素(P<0.05)。结论血清miR-15a、miR-24降低与功能性垂体腺瘤患者术后复发密切相关,二者对功能性垂体腺瘤患者术后复发具有一定的预测价值,且两者联合检测预测效能更佳。Objective To investigate the predictive efficacy of serum miR-15a and miR-24 levels on postoperative recurrence in cases with functional pituitary adenoma.Methods A total of 107 cases with functional pituitary adenoma who received surgical treatment from June 2017 to June 2022 were selected as study subjects,followed up for 2 years after surgery,and divided into a recurrence group(n=22)and a non-recurrence group(n=85).Clinical data of the subjects were collected,and the serum levels of miR-15a and miR-24 were detected by real-time fluorescence quantitative PCR on the 5th days after surgery.The predictive value of serum miR-15a and miR-24 on postoperative recurrence in patients with functional pituitary adenoma was analyzed by ROC,and the influencing factors of postoperative recurrence in cases with functional pituitary adenoma were analyzed by multivariate Logistic regression.Results The levels of serum miR-15a and miR-24 in cases with recurrence were lower than those in cases without recurrence(P<0.05).The AUC of serum miR-15a and miR-24 in predicting postoperative recurrence in cases with functional pituitary adenoma was 0.781(95%CI:0.736-0.831)and 0.819(95%CI:0.774-0.869)respectively,and the AUC of the combined prediction was 0.906(95%CI:0.856-0.951).The single factor analysis showed that the proportion of tumor stageⅢtoⅣand pathological type invasion in the recurrence group were higher than those in the non-recurrence group(P<0.05).Multivariate regression analysis showed that tumor stageⅢ-Ⅳ(OR=2.307,95%CI:1.433-3.715),pathological type invasion(OR=2.140,95%CI:1.266-3.619),miR-15a≤0.52(OR=2.795,95%CI:1.820-4.294)and miR-24≤0.91(OR=2.649,95%CI:1.704-4.116)were risk factors for postoperative recurrence in cases with functional pituitary adenoma(P<0.05).Conclusions The decrease of serum miR-15a and miR-24 is closely related to postoperative recurrence in cases with functional pituitary adenoma.Serum miR-15a and miR-24 have certain predictive value for postoperative recurrence in cases with functional
