摘 要:目的探讨自制标志物引导术中超声在脑深部病变定位和切除中的初步应用效果。方法共纳入脑深部病变患者18例,均行颅脑CT和MRI等检查证实,其中幕上13例,幕下4例,病变为多发病灶同时累及幕上下1例。采用无菌外科手术橡胶手套注入少量生理盐水并用丝线捆扎制成水囊作为标志物,置入脑组织手术创道内,利用超声探查水囊与病变的相对位置关系,引导超声对病变进行定位、路径校正和手术切除。结果本组18例脑深部病变患者利用自制的橡胶水囊标志物引导术中超声定位,均能精准探查到病灶,避免术中探查病变出现“迷路”现象,减少了对脑组织的副损伤;本组病变均完全切除,患者术后恢复顺利,无严重并发症发生。结论利用自制的橡胶水囊标志物引导术中超声有助于脑深部病变的精准定位和安全切除。Objective To investigate the preliminary application effect of self-made marker guided intraoperative ultrasonography in the localization and removal of deep brain lesions.Methods A total of 18 cases of deep brain lesion were included,all of which were confirmed by craniocerebral CT and MRI examinations;Among them,13 cases were supratentorial,4 cases were infratentorial,and one case was multiple foci involving both supratentorial and infratentorial structures.Injecting a small amount of normal saline into a sterile surgical rubber glove and tying it with the surgical silk thread to create a water sac as a marker,which was placed inside the surgical incision of brain tissue;Using ultrasound to explore the relative position relationship between the water sac and the lesion,guiding intraoperative ultrasonography to locate,correct the pathway and perform surgical removal of the lesions.Results In this group of 18 cases of deep brain lesion,the self-made rubber water sac markers were used to guide the intraoperative ultrasonography localization,and all lesions were accurately detected,avoiding the phenomenon of“getting lost”during intraoperative exploration and reducing secondary damage to brain tissues.All lesions in this group were completely removed,and the patients’postoperative recovery was smooth without any serious complications.Conclusions Using the self-made rubber water sac markers to guide intraoperative ultrasonography is helpful for precise localization and safe resection of deep brain lesions.
