摘  要:目的探讨慢性意识障碍(pDOC)患者小脑代谢与意识水平的关系以及小脑-大脑功能连接的特点。方法40名pDOC患者采集颅脑PET/MRI数据,提取小脑9个区域的标准摄取值(SUV),采用线性回归分析SUV与改良版昏迷恢复量表(CRS-R)评分的关系,并基于功能MRI提取时间序列,比较患者与健康对照组间的小脑-大脑功能连接。结果pDOC患者小脑多个区域的SUV与CRS-R评分呈正相关(P<0.05)。与健康对照组相比,pDOC患者小脑VIIb小叶与大脑默认网络及执行控制网络间的功能连接显著降低(所有P<0.05)。结论pDOC患者小脑代谢活性与意识水平相关。小脑与大脑默认网络及执行控制网络之间的功能连接降低可能与意识障碍相关。Objective To explore the relationship between cerebellar metabolism and the level of consciousness in patients with prolonged disorders of consciousness(pDOC)and to demonstrate the characteristics of cerebellar-cerebral functional connectivity of pDOC.Methods Forty patients underwent a 3.0 T brain PET/MRI scan.The standardized uptake values(SUV)of nine cerebellar regions were extracted,and linear regression was used to analyze the relationship between SUV and the Coma Recovery Scale-Revised(CRS-R)scores.Based on functional MRI,time series were extracted to compare cerebellar-cerebral functional connectivity between patients and healthy controls.Results In pDOC patients,SUV in several cerebellar regions was positively correlated with CRS-R scores(P<0.05).Compared to healthy controls,pDOC patients showed reduced functional connectivity between the Cerebellum_7b region and default mode network,as well as executive control network(All P<0.05).Conclusions Cerebellar metabolic activity is related to the level of consciousness in pDOC patients.The reduced functional connectivity between the cerebellum and the default network as well as the executive control network may be associated with disorders of consciousness.
